Local SEO Title-Tag Optimization

Hi Everyone!
A bit of a greenhorn SEO here, and I'm trying to learn a bit more about some of the best practices in local SEO. I'm wondering if anyone can help me with the following scenario:
Business: Dental Care & Surgery
Location: Springfield IN
Name: Springfield Dental Care Experts (example)
Website: 18 content pages, pages dedicated to each service
Since there are 18 pages to work with, there are plenty of places for us to mix-up the title tags. However, I am still unclear as to what the best way would be to do this. I understand that the Geo-modifiers should go at the front, and that the brand name is considered optional.
Would tags such as this make sense?
Springfield IN Dentist | Springfield Dental Care Experts
Nearby town
Mountainview IN Dental Care | Springfield Dental Care Experts
Last question:
The website has pages for each of the services offered by the dentist, ex: Dental Cleanings, Consultations, Fillings, Surgery, etc. Should each of the pages be included in the title tag?
Springfield IN Dentist | Springfield Dental Care Experts - Dental Surgery
My concern is that buy adding this, the title tag would be too long.

Hmm. Yeah, I was planning on diversifying the keywords on each of the pages, using phrases that are getting traffic for a particular area.
There seems to be a lack of well-established "best practices" for local SEO title tags, and this is something I've been trying to nail down for a couple of months. I hear that geo modifiers should go before any keywords, are you suggesting maybe this isn't the best approach?

You should not use the same keyword in all title tags. So this would be the wrong way to do things
Springfield IN Dentist | Springfield Dental Care Experts - Dental Surgery
Springfield IN Dentist | Springfield Dental Care Experts - Dental Fillings
For city specific keywords first make sure they get traffic by doing some research. If they do then you can target those kinds of phrases within the site.
If not I would target generic phrases such as "dental surgery" and have locations pages set up to assist with your geo targeting.

Title tags should be used to give a title for that page.
I would go for (Example)
Page Title - Keyword | Brand Name
So for a contact page, this would be:
Contact Us - Springfield IN Dentist | Springfield Dental Care Experts
Consider a user who is looking at a page full of websites in the SERPS. You want them to easily identify what the page is, as well as the company that the page is for.
Your homepage could be something different, like:
Springfield IN Dentist - Springfields leading dental care | Springfield Dental Care Experts
If this is too long, look at shortening the brand name

Is there any benefit to removing brand name from the title tag?
I just signed up for Moz recently, and have noticed that in my crawl errors, I have hundreds of issues with my title tag being too long. My business is selling prints for landscape/travel/nature photography, and I've built these pages dynamically to where the title tag for pages selling individual photos has the title of the photo for sale followed by a hyphen and then the brand name. The same goes for gallery pages "Gallery Name | Brand Name". Would it be worth it to shorten the title tags by removing the brand name from these pages? Or will that actually harm more than help? Undoing Video SEO
Ok, so here's a good one for you. I was ranking well (2-3rd) position when I got it into my head that I should implement some video SEO to get the video to appear next to my ranking. Well I did that with the help of Wistia. Only problem is my 2-3 ranking become 4-5, and I also became convinced that the page seemed less desirable as video content when I saw it in the rankings. So... I want to undo what I did. I removed the video sitemap and the SEO video markup from wistia, but it's been weeks and Google has still got me listed as video content. Any thoughts on how to accelerate the undoing of this damage?

It looks like there are several title tags on my homepage. This was done presumably to embed titles for pop up windows. This is causing an error report in DMOZ. Do the engines also veiw this negatively?
My homepage has a normal title tag, but when I look in the code I find the developer also added title tags for pop ups within the homepage code. Is this causing an issue with the search engines?

Local SEO - Rich content and list of towns enough?
I'm working on creating pages to target local SEO. I've created pages for example 'wedding band london' with useful content and optimised them with title tags, alt tags etc and will continue to do so for major cities and queries where there are significant search volume. However, I also want to pickup longtail local search queries such as 'wedding band camden' etc... Will adding a list of towns somewhere on the page for each city or county help drive traffic to the site from such queries? Is so what's the best way to structure the page?

Will changing the title tags cause me to lose rankings?
I have a site that gets pretty decent rankings. Based on Seomoz's assessment of my site I have ALOT of title tags that are too long. If I make modifications to my title tags will that hurt my rankings? I am also thinking of removing my company name from the title tag as that is taking up too much space.

Canonical tag help
Hi, We have a product which is marketed by affiliates . Affiliates send referrals to our sale page by adding their affiliate IDs to our product page like http://www.mysite.com/products.php?ref= 12345. We want to avoid the content duplication impression to Google by using canonical tags but we are not clear about its use. Should we use it on http://www.mysite.com/products.php ( actual page) or we should create temporary pages for each referral id i.e http://www.mysite.com/products.php?ref= 12345 and then add canonical tags to all those pages linking to proper page i.e http://www.mysite.com/products.php ? Assume a brand new blog being designed and all other things equal. What are the pros & cons between using the url structure /blog/content-title vs. /category/content-title? Note:
