Domain.com isn't recognized by on-page-grader, but domain.com/index.php is

I am running a website through On-page-grader, as www.domain.com and scores an "F" for a specific keyword. When it's ran as www.domain.com/index.php, it scores an "A" for that same keyword and has everything checked other than "keyword in the domain name".
There are no other files such as index.htm, or index.html that would interfere and can't figure out why this page is not being recognized.
I checked, the robots and .htaccess file, but do not see anything that would hinder. Could this be a server issue?

I posted this to the .htaccess file and it now works.

Hi, domain.com & domain.com/index.php are regarded as separate pages so I would assume from your question that you want the index.php to be your root page? This can be achieved via Directory Index using .htaccess
You should see something like this in the htaccess file: DirectoryIndex index.php index.html

Will Removing or Disavowing Toxic Links Improve MOZ Domain Authority?
The vast majority of the 140 domains that link to our website are very low quality directories or and other toxic links. Only about 20-30 domains are not toxic (according to Link Research Tools confirmed by out manual inspection of these links). Would removing some of these links improve of MOZ Domain Rank? What if we cannot remove them, can NOZ detect a disavow file? In general would improving the ratio between good quality and poor quality links improve domain authority? Thanks,

Crawl report shows that it gets 4xx errors for pages that work fine. Why?
On the crawl report it has all these "Critical Crawler Issues". They all say "4xx Error", yet when i click on the link from the crawler report, it goes to a perfectly functioning page, not a 404 page or anything. If i click in it actually says it's a 403 error. It's all for pages generated by the IDX solution for our real estate website. Is Moz broken or am i missing something? Keyword used 2389 times in page
in the "on page grader" tool I get that the keyword appears 2389 times. I'ave searched for it in the developer console, thinking it might be due to invalid HTML - but the number it's appearance is no near to 2389. In Moz Medium Pro how many pages can you track for keyword rankings at a time? I know you can track up to 50 in Standard.
I would possibly like to upgrade my subscription from Standard to Medium because I want to be able to track more pages in Rank Tracker. Unfortunately, the product descriptions do not include how many pages you can track in Rank Tracker.

We Launched a new site and Rogerbot is still reporting on links/errors from the old site, is there a way to clear those out?
We are mostly a Branding agency, and have not put a lot of effort into SEO for ourselves... SEO tends to take a backseat to design most of the time, making it a little difficult for me at times when it comes to SEO. We recently launched a new site, http://Roninadv.com/ and the developer and I have done quite a bit of work to make it work well for Google. I was really looking forward to a new crawl report from Roger, but alas, It's like Roger crawled the old site? The new site has been up since last Monday. Is there a way to clear out the old errors? Do I just need to give roger more time?

On-page Grader and Cyrillic keywords
Hi all, I'm trying out the On-page grader and i see "funny" output when it comes to cyrillic keywords. I can see 73 occurrences in the title, 108 in the meta description tag, 34 in the H1 tag, etc. etc... Of course, these are not correct and i've manually checked. I've tested a few other pages in Bulgarian, using Cyrillic kewords... same story 😞 Please, let me know if there's something i'm missing, as this is crucial for my work. Thanks in advance.
