Page Title & Meta Description Getting Cut Off In The SERPs

Hi Guys,
I am trying to figure out why my page titles and meta d tags are getting cut off in Goofle SERPS.
My page titles are 70 characters or under (including spaces) and my meta Dd's are 155 characters or under (including spaces) so I cannot work out why They are getting cut off.
Is there something I have missed?!

I read a couple articles about the issue a while back. Search engines do not actually use a hard number of characters in both the title and the descriptions. They actually use a pixel width. Do You Add City Name & Key Word For Every Page?
Hello, I'm new to SEO but feel I have a decent grasp on it. However, I had a question pertaining to key words and using my city name in it. For instance, if I am using the key word "herniated disc treatment" do I need to put in my city name behind it or does google recognize that I am already in my city area because of my geo tagging and having it listed on the footer of my site? Use of '&' in meta title
Hi, I know that use of '&' would be helpful to save space and also add more keyword variation to the title tag. But just want to make sure if it matters if I use '&' in most of my title tags? And also is it common to use more than & in one title? Would the following title be different in Google's perspective regardless of the title length? I am thinking they are all targeting the keywords 'fruit cake' and 'fruit bread', but the first one is the best. buy fruit cake & bread buy fruit cake & fruit bread buy fruit cake and fruit bread Thanks in advance.

Business Name is Meta Description
I would like to know what your opinion would be regarding the business name displayed in the meta description. Would you write your business name as: Business Name or BusinessName™ (no space with Trademark) I used MOZ example from here (Meta Descriptions Best Practice) and inserted the different business names. Welcome to Business Name in San Diego, California - the nation's largest urban cultural park. Home of 15 major museums, renowned performing arts venues... How to overcome blog page 1, 2, 3, etc having no or duplicate meta info?
As the above what is the best way to overcome having the same meta info on your blog pages (not blog posts) So if you have 25 blog posts per page once you exceed this number you then move onto a second blog page, then when you get to 50 you then move onto a 3rd blog page etc etc So if you have thousands f blog pages what is the best method to deal with this rather than having to write 100s of different meta titkes & descriptions? H2's vs Meta description
in some of my serp results the h2's are showing up instead of the meta description. i have read that H2's arent really valid anymore. can someone clarify this for me?

Punctuation at the Start of Page Titles
one of my clients appears to be using an exclamation mark (e.g. "! Graphic Prints By Mirrorin - Fun Childrens Graphic Prints") and to be completely honest, I have no idea if this is bad practice or if it wont have any affect from an SEO point of view? Any help would be appreciated because it is site wide, therefore if it is an issue I would like to be able to get it sorted asap! Thanks

Does a phone number in the title tag hurt your rankings in SERP?
Hi Mozzers, One of my client is a carpet cleaner and I was wondering if adding a phone number into the title tags would hurt our rankings . I am asking because the client has mentioned it and that we do have some space to add a phone number into the tag. Thanks
