Word mentioned twice in URL? Bad for SEO?

Is a URL like the one below going to hurt SEO for this page?
/healthcare-solutions/healthcare-identity-solutions/laboratory-management.html
I like the match the URL and H1s as close as possible but in this case it looks a bit funky.

Like others have mentioned, if your site has both the "healthcare solutions" and "healthcare identity solutions" categories (category and subcategory), then it would make sense to keep it that way. We should be able to navigate to each of those pages in the URL.
If you just are referring to the URL, though, the keywords in the URL is such a small part of the search engine algorithms that it really won't make a huge difference--and I hardly believe you'll be penalized for it.

It does make navigation sense considering how the site is laid out as we do have other healthcare-specific solution categories. Thanks for the insight. Paralysis by over analysis in SEO is a real thing.

Assuming it makes navigational sense, I don't think there is a problem. If you have other healthcare solutions beyond just identity, it would make sense.

Is it bad to update product titles and URLs if they are only slightly modified
I am doing some house cleaning on the site and made some minor updates to product titles and a rule was written in and it auto updated the URL to what the product title was with a redirect put in place from the old URL. If this a bad thing and should i leave the URL alone and just update the product title? Then for the ones i did change the Product title and the URL was updated is this a bad thing and should i have just left the URL alone? These are all high ranking popular products so dont want to mess with any rankings going into busy season?

Old URLs Appearing in SERPs
Thirteen months ago we removed a large number of non-corporate URLs from our web server. We created 301 redirects and in some cases, we simply removed the content as there was no place to redirect to. Unfortunately, all these pages still appear in Google's SERPs (not Bings) for both the 301'd pages and the pages we removed without redirecting. When you click on the pages in the SERPs that have been redirected - you do get redirected - so we have ruled out any problems with the 301s. We have already resubmitted our XML sitemap and when we run a crawl using Screaming Frog we do not see any of these old pages being linked to at our domain. We have a few different approaches we're considering to get Google to remove these pages from the SERPs and would welcome your input. Remove the 301 redirect entirely so that visits to those pages return a 404 (much easier) or a 410 (would require some setup/configuration via Wordpress). This of course means that anyone visiting those URLs won't be forwarded along, but Google may not drop those redirects from the SERPs otherwise. Request that Google temporarily block those pages (done via GWMT), which lasts for 90 days. Update robots.txt to block access to the redirecting directories. Thank you. CSS background image links bad for seo?
On one of the websites I manage SEO for, the developers are changing how our graphical links are coded. They're basically coding in such away where there is no anchor text and no alt tag, so for example: So there's no anchor nor alt context for Google's crawler. How badly will this affect SEO, or is it extremely minimal and I shouldn't worry about? Thanks in advance.

I'm researching Schema opportunities for a bank, but besides the shema markup available today (like bankorcreditunion) and developments with FIBO, I can find no answer as to the effect of tagging interest rates and such in terms of SERP/CTR performance or visibility. Does anyone have a case study to share or some insight on the matter?

How do I deindex url parameters
Google indexed a bunch of our URL parameters. I'm worried about duplicate content. I used the URL parameter tool in webmaster to set it so future parameters don't get indexed. What can I do to remove the ones that have already been indexed? For example, Site.com/products and site.com/products?campaign=email have both been indexed as separate pages even though they are the same page. If I use a no index I'm worried about de indexing the product page. What can I do to just deindexed the URL parameter version? Thank you!

Special characters in URL
Hi There, We're in the process of changing our URL structure to be more SEO friendly. Right now I'm struggling to find a good way to handle slashes that are part of a targeted keyword. Is link cloaking bad?
I have a couple of affiliate gaming sites and have been cloaking the links, the reason I do this is to stop have so many external links on my sites. In the robot.txt I tell the bots not to index my cloaked links. Is this bad, or doesnt it really matter? Thanks for your help.

Should me URLs be uppercase or lowercase
I'm in the middle of doing a bunch of 301 redirects for me site. Should I make them Lowercase, uppercase, or does it matter? Also, do I want to be using hyphens (-), or underscores (_)? Any other tips? EX: http://www.stupid.com/golf-slippers.html OR http://www.stupid.com/Golf-Slippers.html
