you still using meta-keywords ... they can only help the competition not google position.
meta - description tags are all the same for different pages.
If yes it can be a main reason. You have to diversify descriptions and they have to be consistent with title and page content.
e.g. for <title></span></span><span style="color: #5e5e5e; font-family: Helvetica, Arial, sans-serif;"><span><span> </span>Buy LED lighting at the best price.</span></span><span style="color: #5e5e5e; font-family: Helvetica, Arial, sans-serif;"><span></title>
in description use: name="Description" content="LED lighting is the latest craze. ..... an so on about LED up to 140 characters">

For quick checks I use on iPad SEO RANKING by Leon Huang. I have tested many and that one is the most accurate. In Poland we have many sites with very good tools but they do not have English UI

The simplest example of semantic markup are H tags (H1, H2 ... H6), other ... bullets
Non semantic tags are e.g.
Since late 90' semantic markup (and web) are strongly supported by CSS which is responsible for presentation (look) of the web.
So ... summarizing everything what is an element of structured document is semantic markup and everything what is neutral for document structure is non-semantic.

I agree with EGOL and Gordon. What I want to add ... think that internal consistency of the site is very important. Good content structure (H -tags, content with keywords) - title - description - url should be the first elements of the subjects of your mind.
Also very important is competitors tracking. What do they have that you do not have? Maybe they follow some (local ... global) trends? Social impact is another area to check - do you have many G+1, FB shares, votes etc...

When did you submit these 455 url's. Usually it take few weeks before Google index pages.
Sometime "fetch as google" help. Also please check in menu Health -> Index Status -> Advanced how many url's are not "selected".
Other factors:
good xml or xml.gz sitemap
unique url's
external backlinks to url's

Is your tags and categories are unique? I mean unique queries ... different content. You have to use tags carefully, differentiate them in posts.
Usually such a situation take place when the same tags are using for many posts and a tag query is the same. Also you should create good structure of a categories and draw attention to categories and tags were not identical. Good strategy is to build a category tree and good tag structure.
Category - series of articles on the same topic/subject
Tag - unique connection, relational table, describes or defines some aspect of an information
Example.
Categories (New York Times):
- News
- - World
- - U.S.
- - POLITICS ...
etc.
Tags are usually use in searches or tag cloud:
Barack Obama, Apple, Natural Gas ... etc
It is pretty easy in WP blog in Posts menu. remember to write a description for each tag and category.
If you can't do it now to not lost earlier SEO work you can still use a solution described by Shane answer.

This is you have the same content for different url's. Default state of the form - form not filled - always redirect to first part no matter what url you type. REL canonical doesn't help in such a situation.

Keyword rank in google depends on many factors. PA, DA and others are only measures using to compare specific factors. You can read about them here: link and other MOZ Metrics from right-menu.
It is possible that page with really poor measures/ranks have good position in SERP's. If you have niche searches, one good article can shoot up in the rankings.

According to Matt Cutts, Google doesn't penalize sites just for having multiple H1 tags, see: link
In my opinion they crawlers/indexers can be set to detect overuse of H2 tags and it is possible that Google can penalize for that. From usability point of view and good structured content I fully agree with Matthew.
And from SEO perspective ... how can you tell crawler bots what is most important on your page? Consistency of main factors: meta title - meta description - keyword - title (H1) - content structure an so on is crucial for your page.

I usually use full sentences including a keyword. Have tons of searches for these elements. It is a long tail but it increases site traffic.
In my opinion optimal size is like in the meta - title ... no more than 70 characters.

2. Social impact ... really low (only 4 G+1)
3. Your position (6th in google.com) for keyword "expedite us passport" is for the page http://www.passportsandvisas.com/passport/index.asp
4. Many of good places are images not text containing keyword (H1, H2 ...), like "Looking For US Passport .... - image
bullet list below ... image, In my opinion it is a best place for main slogan in H1.
5. keyword stuffing in meta description "Expedite US passport and travel visa service : Expedited US passports ( new , renewal, lost, child ) : Nationwide Service" ... it is not a description ... it is stream of keywords

No mobile css ... but it has low impact
Increase Text/HTML ratio

In my opinion CSS is "The Best". Simple, easy usage, easy changes, very good speed of page load ... etc...
As I red on many forums on the internet JQuery and JS are are available for robots, so there are no contraindications to employ them.
But
CSS - better code/text ratio (no long scripts in page code)
CSS - simple usage and changes (CSS3, HTML5)
CSS - faster loading (only simple text and html)
In my opinion, now when we have HTML5 and CSS3 there is no better way - it's innovative and simple solution,

On the right side of a keyword you can see the button with label and with "x" sign. Just click on "x" and that's it.
Other way to delete a label is - check the keyword (tick) and from menu Add/Clear Labels tick label and next click Remove labels

In addition to my last response. WP templates (themes) have nothing to Google Page Rank.
Template can have good design, helpful admin page, good additional features, widgets etc... It can be very good for call to action solutions, eye-caching features ..... And the rest is your continuous work.
Page Rank is calculated factor based on link power, number of links, link structure and many more...
For general view of PR you just read an article in wikipedia - http://en.wikipedia.org/wiki/PageRank
Don't think (or dream) about high PR, rather go out to meet the expectations of [your site] visitors and adjust to what your customers want ... and be active

I think you all right but ... I have tried many WP templates and can say that some of them works better then other in SEO. There is no straight recipe to choose good one, having in mind other than SEO features or design.
CMC-SD wrote about Yoast experience with SP themes ... I also don't trust.
Don't use templates with build-in SEO module, I haven't met even one good. Choose good design, useful features, check code validation (W3C) and test & try and so on...
Large selection of WP templates you can find on Themeforest ... but some of them are good - some not.

In my opinion 1 client = 1 Google account (GWT, G+, GA ...etc). I agree with Davinia that linking accounts is good solution to make your work easier...
Having one account per client let you transfer whole data to client after contract termination and the client will be satisfied.

in my opinion you should 'no index' sites with high bounce rate compare to short time of visit (less than 10 sek). It's true or real bounce rate - visitor spent less than 10 sek on the one site and then left website.
Also good idea is no index or even delete posts with 'heavy' content with long time of page load ... as I think flash galleries could be heavy. Page loading time (long) could also increase bounce rate.
So you have a choice to have 600 posts or ... maybe 100 good posts ... good content vs average or poor content.

Cesar I think your problem is not different order of keywords but duplicated content. If you have two websites with the same content it can be a problem and it was probably the reason of Panda influence on your website.
let me understand, do you have: http://www.example.com and http://example.com with the same content?

http://www.seomoz.org/pages/search_results?q=www+or+non+www
In my opinion you should choose better site (www or non-www) by checking a value in OSE and finally do a 301 redirection of lower ranked to higher ranked and do the same in GWT ...that means choose higher ranked as your primary.

Yes, in my opinion you should submit languages. I'm not sure but think that main site on .com should be set as basic NL site.
For each sub-site (language ver) sitemap in particular language is needed, I have no idea how your sitemaps are created. If on automatic way (plugin or other way) it seems to be a bit complicated, but it's better to have sitemap than not to have, it is worth your effort.
Use meta tags "alternative" in section on each site.
And ... ask other people, maybe there is something new. Theres a lot of stuff in the GWT blog and forum...

I'm not a prestashop specialist but i know that on one licence you ca do something like that shop-en shop-cs - it is from Polish prestashop forum ... forum.prestashop.pl
My " use rel="alternative" for language versions" refers to the situation when you have language versions and you want to prevent from duplicated content.
When it comes your situation - rankings, in my opinion it is better to have 3 different subdomains or subfolders for different languages.
As I understand your site uses language switcher... and a versions are in main domain subfolders. For GWT doesn't matter which method you use - subdomain or folder.
Key thing is that you should submit to GWT 3 different language versions (subfolders) and do a verification with different GWT meta tag (if you use this method for GWT verification) or code or with three diff GA accounts.
So 3 lang ver -> 3 GWT "sites" -> 3 verifications by GWT
And last but not least ... you still should use "alternative" tag to ensure crawl-bots that it is different site version not duplicated content. remember that not only text can be duplicated content... images or other stuff too

Firs of all what are your server and site settings? There are many tools to check it. If you have www and non-www you have duplicate content and probably thats why you lost on rankings.
For all my sites I used without www address and redirect everything *.example.com (all subdomains) to example.com (respectively http://www.example.com or http://example.com).
Why ... theory is internet behavior.
1. It's obvious that in present times more people search directly writing a words to search engine address field rather than write in google search-box field. So if you have a good brand it is more likely that they write your branded name than your branded name with the www in front of branded name.
2. www is a subdomain of a main domain - there's no point to guide people to subdomain.
3. In server configuration *.example.com with everything in subdomain will direct traffic to example.com
4. It is simple and short, now everything going to shortage and simplicity, why not site names?
5. Like in point 1 but bit different - it's mostly young users behavior to shortage addresses so treat 5 as a reaffirmation of 1 dedicated to the future

As you wrote - you did change nothing. It is possible that one keyword still has a good support for content, links etc... but other not.
Your competition probably did changes. Few tips:
you should analyze bounce rate in correlation with time spent on page
for pages with low bounce rate and short time spent and low traffic is better do them noidex and nofollow - they decreases site quality
implement microformats,
- big impact has rel="author", "author" links should provide to author's site not to publisher site
social media are the key to modern SEO position
G+1 is not a social media but it helps a lot
employ responsive site content (mobile) more than 20% of the internet traffic is on mobile devices (usage of CSS3 and HTML5 is a need)
use rel="alternative" for language versions
use Google Map Maker
and many many more, I advice you to read mentioned conference materials
You have to change your site - it is a must.
Just imagine that you start on Ford Model T in Formula 1 ...

Maybe it was because you did nothing, remember the words "Change or die" said in 4th century by Claudian Claudianus and also in recent times by marketing guru John Porter from Harvard.
In 2011 google did 525 changes in algorithm. In 2012 they are still doing changes. Google new trend is to promote sites friendly for users not for robots. Backlinks are still have have impact but not as big as in the past. Now we're stepping in new world ... semantic search. Now for Google good article is article between 1000 - 2000 words with good quality and with minimum once a week publication on your site/blog. There are also lots of other changes and news ... you should take a look on SMX conference materials.
...so maybe you have to change something, do a site revision and next do changes...

First four links are 404 error with content:
"Not Found"
"Apologies, but the page you requested could not be found. Perhaps searching will help."
... so you have duplicated content... because it is the same
fifth url
returns good content ... but sixth is redirection to fifth.

Agree with Devin, meta description is very important but not major factor but I've noticed that in question head you asked about TITLE tag.
Title tag is major SEO issue, so if you can employ even automatic meta tag generator.
If not ask programmers to export all meta title and description to excel sort by URL and set unique title and description and next ask your tech staff to import it to the blog. I hope it is not dynamic generated content if yes only first solution is possible.

GWT has new panel, and sometimes it shows such strange communicates in sitemaps section. But in GWT dashboard you can find Sitemaps widget and a number of indexed sites "URLs in web index". The number of url's in this widget is accurate to "reality"
I don't know your market but in my 95% of people will write "carpet cleaning" instead of "carpet cleaning my city" - now in google places and localization is not a point to emphasize "my city"
About keyword stuffing - I'm not sure - probably not.
your site: If you think like a lot of people what is their need? order and simplicity. You can read book about site usability written by Steven Krug "Don't make me think" - it's my usability bible. (site: sensible.com)
And I suggest buy better photos from stocks or employ a professional photographer to take few very good pictures and use it on page. Photo can tell more than 1000 words
Sorry for non SEO advise, but the truth is that SEO is only way to attract customers, next you have about 5 seconds to keep him on the site. Bounce is a loss!

Use simple and understanding sentences, without unnatural forms, remember good marketing is not closed in the word "BEST" "Very BEST" etc... I usually avoid companies with such a communication.

I think that depends on what is the position of your site now. If it's good there is no pressure to do changes in other case you can take a risk. In my opinion good structure of content help in SEO an if I were in your situation i would take a risk of change.
