As Francisco says, Danny Dover's book is a great resource. However, even though it contains two detailed example SEO audit reports, it doesn't contain an example SEO proposal. Neil Patel has a useful Feb 2013 post about what to put in a great SEO proposal here:

There must be a tool which could provide an overall layout and structure for the SEO proposal. Seomoz doesn't have one. It is there in Raven. Do let me know if found anything. Thanks

I think is worth it to develop the proposal in a custom manner for each client based on his needs, KPIs, current status and so on.
We usually touch the bellow point in each proposal:

general status of the domain and general guidelines on what can be improved
-
on page optimisation improvments (from url structure, code, ux, nine relining, meta tags and content
-
link profile analysis, improvement channels
-
social media in correlation with SEO.
depending on the client and status of the proposal (initial, strategy etc) you can get into details with all or some of the points.

Hi guys, I hope you're all doing well! We're a small personalised gifts company who specialise in the provision of phone cases, mugs, macbook covers and the like. I head up the Digital Marketing but have little experience in the technical side of SEO and have very limited resources in terms of budget and staffing. Over the past few months, I've been working on stripping down the thin content on the site, fixing duplicate content issues and focusing on other digital channels to boost revenue. However, as of recent we've noticed a significant drop in traffic and our rankings. Onsite SEO vs Offsite SEO
Hey I know the importance of both onsite & offsite, primarily with regard to outreach/content/social. One thing I am trying to determine at the moment, is how much do I invest in offsite. My current focus is to improve our onpage content on product pages, which is taking some time as we have a small team. But I also know our backlinks need to improve. I'm just struggling on where to spend my time. Finish the onsite stuff by section first, or try to do a bit of both onsite/offsite at the same time?

Domain Forwarding - SEO Impacts?
I have a site that has been active for years - thinkbiglearnsmart.com. Awhile ago I had purchased about 50 domain names that were relevant to my company. I still have those urls and would like to use them to point to different pages on my site - just because they have good key words in the URLs. For example - one is dreamweavertrainingclassesonlinelive.com. Currently they are all redirecting to my homepage. A. is that hurting me? B. I would like to redirect to the more relevant page. ie the page dedicated to Dreamweaver training (http://thinkbiglearnsmart.com/dreamweaver-creative-cloud-training-course/ ) Will this hurt my Dreamweaver keyword for example because there is already a 301 redirect on that page from a very old Dreamweaver link which was something like thinkbiglearnsmart.com/dreamweaver C. On my hosting account where I can select where the URL forwards to - it has an option for "Location forwarding" and "Frame forwarding" - currently they are set to Frame forwarding - which one is best? Any help is much appreciated!!! Thank you!

SEO Checklist for eCommerce?
Over the past week or so, my eCommerce site has experienced a large drop in organic traffic from Google.I have used analytics to determine that the pages that have seen the biggest losses in traffic are product pages. I know there are so many seo checklists/site audit checklists out there recommending different things, but does anyone have a good checklist (or link to a good checklist) that would fit our situation best? What are the SEO implications of a CNAME?
(please ignore ridiculousness of hypothetical situation) Lets say Amazon had a food division which was at food.amazon.com. I partnered with Amazon's food division and now food.amazon.com will point to my website (food.com). Amazon adds a CNAME record so food.amazon.com resolves to food.com. If food.amazon.com has built up significant page rank / domain authority, will food.com be getting those benefits? Also, lets say food.amazon.com/rice has a lot of PR / authority -- will food.com benefit from the value of those internal pages?
