Domain.com and domain.com/ redirect(error)

When I view my campaign report I'm seeing duplicate content/ meta for mydomain.com and mydomain.com/ (with a slash)
I already applied a 301 redirect as follows:
redirect 301 /index.php/ /index.php
Where am I messing up here?

Actually I figured out all of the problems, here is the solution that worked for me:
RewriteEngine On
RewriteBase /#Rewrite bare to www
RewriteCond %{HTTP_HOST} ^domain.com [NC]
RewriteRule ^(.*)$ http://www.domain.com/$1 [L,R=301]
RewriteCond %{REQUEST_FILENAME} !-d
RewriteCond %{REQUEST_FILENAME}.php -f
RewriteRule ^(.*)$ $1.php [NC,L]

There are a couple things you can try here. I assume you're using .htaccess for the redirect? I'm not an expert on these, and they can get messed up pretty quickly, but I'd start by listing the full url in the second instance, like this:
Redirect 301 /index.php/ http://www.example.com/index.php
Another easy way to solve this problem is with a canonical tag. Just make sure both pages have a duplicate canonical tag in the section that points to the same target, like this:
To be honest, although these are technically duplicates you are dealing with, Google has publicly stated that trailing slashes aren't that big of deal, so if you can't invest the time in fixing it, it probably won't hurt you that much.

Yeah I re-crawled it after applying the 301. Here is my full htaccess:

Supposed to redirect from nonwww to www, hide .php from pages to look clean: domain.com/category vs domain.com/category.php Then finally redirect index.php/ to index.php

After you fixed the issue did you wait till the next report was generated to see if the crawl detected the same error?

Www to non www on a .com/blog url
hi guys, I have had to reset my site from www to non-www. via htacces and this worked out just fine.However, the /blog WordPress section will not redirect to the non-www. Redirect if old browser or OS to other domain?
Hi, I use cloudflare free SSL option. All is fine except it wont work with windows XP . How do you think is it ok in case of old OS/Browser redirect visitors to other domain or subdomain? Increase in authorization permission errors error after site switch

OK to block /js/ folder using robots.txt?
I know Matt Cutts suggestions we allow bots to crawl css and javascript folders (http://www.youtube.com/watch?v=PNEipHjsEPU) But what if you have lots and lots of JS and you dont want to waste precious crawl resources? Also, as we update and improve the javascript on our site, we iterate the version number ?v=1.1... 1.2... 1.3... etc. And the legacy versions show up in Google Webmaster Tools as 404s. Why is it that when I run an "On Page Optimization Keyword Report" for my website I get a different score when using http://www.tandmkitchens.com vs http://tandmkitchens.com. For a UK business where there is no .co.uk or .com opportunity, is it better to have a .net address or would .uk.com be better? Or no difference...This company is UK focussed only.
We have a niche keyword domain possibility but I am not sure which way, if any, is better. The .com and .co.uk options are not available but there are various other ones - including .org, .net, .uk.com. Is there any domain/seo benefit of one version over another? Any thought gratefully received..
