What is the biggest mistake you made with one of your SEO campaigns? When did it happen? What were the consequences and what did you learn from it?

I'd have to second this one. Hiring an oversea's copywriter because the client insisted on getting the cheapest one available.
It was disaster. I spent more time fixing and correcting the copy for grammar and SEO than they spent writing the content.
After about a month the client agreed, and we hired a favourite local copywriter of mine who nailed the copy first time. In the end she only cost a little more and because she could meet the client, we formed a great team and work to this day together.
I put that down as a reminder that I won't work with an overseas writer again.

Oh I have been this route before. In house writers I found are a must.

Biggest mistake?
Hiring a content writer overseas, even though that person was "high rated" it was utter crap. Got an in-house writer about 2 weeks after that.

May be out of many websites I started with the most important one and made links. And some of the links were low quality ones resulting in bad rankings.

Generally our biggest mistake is not understanding our clients correctly. Some will approach us and tell us that they need to rank terms to get traffic to their site which is normal. Some tell us just to get traffic to their site. We have found that we have to settle on keywords no matter what, because even if we get thousands of real visitors to a site, it really matters what keywords we are using to a client regardless if they are still relevant. I have had clients get mad that we weren't ranking them for terms like "toys" for example for $325 a month.
We learn on a daily basis on how to better serve our customers, but something new always comes up.

Which one is better?
We are creating a new website and got stuck while deciding the URL structure. Our concern is which url is better in terms of SEO i.e. pune.fabogo.com/spa or fabogo.com/pune/spa and why. Also which one would rank faster if someone searches for spas in pune if both pages are same.

Repeatedly target a rolling list of kws..or is that cannibalization? Biggest Confusion in SEO Ive found
Also suggesting a WBF topic. Ive read and researched with no luck here... would love a Moz staff reply too! Is it better to blog repeatedly on the same topic (writing multiple blogs around the topic of "content marketing" for example in hopes Google sees you as an authority on the topic over time) OR is this keyword cannibalization? Is it better to have one powerful and comprehensive page on a topic if it makes sense. Thanks!

SEO Experts: Where did you start?
Hi all! I'm beginning to travel down the road of becoming an SEO expert! I've attended the latest few webinars on Moz, and have started watching the White Board Fridays. However I'm wondering, for the current SEO Experts, how did you get to where you are today? I.e. What books did you read? Did you pay for classes or just learn everything from Moz? Where is a good place to get an SEO Expert Certification/Is it necessary? How long did it take you to become an expert? Link-building best SEO practice (one-off VS periodic blogging)
Hi all, Generally, what would be best when building a website's ranking through link building? Having the same links from the same bloggers or receiving new links from different bloggers every time? A lot of the SEO services offer 4-8 blog backlinks per month. Would it be best if these links came from different sources every time or most from the same sources each month? URL Shorteners. Are they SEO Friendly?
Do URL shortener services like bit.ly act as 301 redirects? I was thinking about utilizing one for longer query based URLs and didn't want to risk losing link juice. Thanks for the insight! Regards - Kyle
