Optimizing web page

I have implemented most of the suggestions, which are offered threw SEOMOZ on our web site www.zaposlitev.net. But rankings are not improving.
Could anyone help please?

And to expand on Kevin's answer.... results in whitehat SEO (i.e. following best practices) can sometimes take months before it really kicks in. We're working towards the long haul not the short game. Once you have all of your on-page/on-site stuff done, now's the time to start looking at relevant links, guest post opportunities, outreach, social media, etc. And then you can come back to the on-page/on-site stuff, see how its been working, determine what else can be tweaked/fixed/changed, etc. and continually update your site(s) with new, fresh content on a regular basis. Lather, Rinse, Repeat.

The SEO process is never complete, but always a work in progress. Just give it some time and work in getting some good contextually relevant inbound links.

It's not possible to add a keyword to the homepage url. Do we still add a secondary keyword along the primary one on the home page title and meta description etc. How do we make our primary keyword to dominate in this case. Thanks

How many pages should we optimise?
I have more than 250 pages on my site including my products. Is it a good idea to optimise each page with a unique keyword or is there a limit to the number of pages we should aim for?

Home Page Keywords not Ranking and Assigned to Inside Pages
Hi, thank you for taking the time to read this. We have a few websites with the same problem. I will use http://www.prepared-meals.com as an example: The home page was ranking on page one for keyword "Prepared Meals". The site is about 6 months old. We use the Moz page optimizer on all pages of our websites to score an A rating. Recently we found the home page is no longer showing up in search results and the keyword "prepared meals" now points to an inside page that is not relevant: http://www.prepared-meals.com/Senior-Meals/Moms-Meals-Reviews.html this page shows up for Prepared Meals around page 15 in Google results. We have read keywords in the URL might be the issue, even though the page optimizer in MOZ says to do that. We are wondering if this is the issue or there is some other problem we are not aware of. Over Optimization - What exactly is it?
I keep hearing this term "over optimization" and people assuming they have received an over optimization penalty. I also hear people say, "You need to optimize every page on your site." So at what point does a site become "over optimized?" Are there any good reads around here that explain this better?

Just trying to get more feedback - so we recently edited title and meta descriptions for existing website SEO and we've noticed in the past several weeks, our client's website has dropped out of the top 50 in a variety of terms we were targeting that they used to show up for (note: when updating SEO, we DID NOT remove any relevant terms we were targeting). When the website does come up in searches, it is the old meta description and title. So far, the feedback we've gotten is that first, it takes Google a few weeks to recrawl and index - however, we are now on week 3 after the changes and still no rebound in rankings. We were also told to check with the SEO Moz page grader to be sure the keywords were being optimized correctly - got As and Bs for the test terms I tried. We also submitted an XML site map to speed up the crawl process as another user suggested. We've tested the site with various tools to make sure there are redirect errors, etc. and everything looks fine. Again, it's now been 3+ weeks and no ranking rebound. Any other suggestions on what could be happening?

Third party pages
Suppose you are using a third party tool such as an affiliate program. Typically, all the files are organized under one subdirectory. In addition, you may have little or no ability to modify any of the files in terms of SEO. Would you recommend hiding the entire subdirectory with a noindex? Best,
