Home Page Keywords not Ranking and Assigned to Inside Pages

Hi, thank you for taking the time to read this. We have a few websites with the same problem. I will use http://www.prepared-meals.com as an example:
The home page was ranking on page one for keyword "Prepared Meals". The site is about 6 months old. We use the Moz page optimizer on all pages of our websites to score an A rating.
Recently we found the home page is no longer showing up in search results and the keyword "prepared meals" now points to an inside page that is not relevant: http://www.prepared-meals.com/Senior-Meals/Moms-Meals-Reviews.html this page shows up for Prepared Meals around page 15 in Google results.
We have read keywords in the URL might be the issue, even though the page optimizer in MOZ says to do that. We are wondering if this is the issue or there is some other problem we are not aware of.

Hi Craig,
To me it kinda looks like an 'Exact Match Domain' issue and that over this year's algorithm updates it may have now flagged your domain as of low quality. The fact that your site is very young as well and that this looks like an EMD for your target search query it's likely it is now going against you. There is low historical data that the search engines can go on so it's likely to be devaluing your site to searchers.
Ref. "We have read keywords in the URL might be the issue, even though the page optimizer in MOZ says to do that. A page's URL yes is used as an indicator of a page's relevancy but this is more about domain name address name rather than a page's URL.

I think Google has penalized the site for over optimization or trying to trick google for the keyword 'prepared meals'.
Looking into the backlinks you have via keyword 'Prepared Meals' - sample here http://prntscr.com/1yzst1, i notice that they are from sites which are blog networks that commercially sell backlinks. If that is the case, it is not a good practice.
I'd propose that you check the backlinks and take corrective measures.
Other than this, I cannot find anything else that would be the cause for the same.

Will it upset Google if I aggregate product page reviews up into a product category page?
We have reviews on our product pages and we are considering averaging those reviews out and putting them on specific category pages in order for the average product ratings to be displayed in search results. Each averaged category review would be only for the products within it's category, and all reviews are from users of the site, no 3rd party reviews. For example, averaging the reviews from all of our boxes products pages, and listing that average review on the boxes category page. Duplicate home page URL on crawl test
Hi i just recently made a crawl test but before doing that i made sure that i have no more duplicates on my site i am using joomla and as of now i only have 11 links on my site but when my crawl test is done i saw duplicate url of my homepage the duplicate url has a trailing backslash so basically i have all the 11 links + 1 duplicate URL http://mangthomas.com http://mangthomas.com/ can you guys give advise how i can remove the duplicate i dont even know which one to retain. On-Page Report Card: Whats up with the TITLE of the page?
Started to fix the SEO issues on a customers website. When I run a "On-Page Report Card" It says that the title of the webpage:

www.visbymaklarna.se/visbymaklarna.html Is "visbymäklarna - Ditt förstahandsval på gotland." But if I check in the source code of the webbrowser the title should be:
name="title" content="Vi är mäklarna på Gotland som sätter människan i första rummet" /> (Actually this is with special encoding for the swedish characters. The title in coded text is: "Vi är mäklarna på Gotland som sätter människan i första rummet") Anyway the title of the webpage source code and the title of what SEOmoz reports is different. Is it necessary to add keywords to all of your pages?
Hi Everyone he company I work for has just built a new website with approximately 87 pages/sub pages. Should i be looking to add keywords and descriptions to all of these pages, via the allocated areas in the back end of the site? I am using "google's key words" tool to generate relevant key words. Is it wise to target different keywords for each page?
I am running a dating site, I am trying to focus on 5 keywords on the index page. My Search page is one of the 4 other pages on the site. My question, is it wise to target different keywords on these pages or am I better off focussing on the same 5 keywords as the index page? Thank you,

E-Commerce product pages that have multiple skus with unique pages.
Hey Guys, With the recent farm/panda update from google i'm at a cross roads as to how I should optimize product pages for a project i'm working on for a client. My client sells tires and one particular tire brand can have up to 15 models and each model can have up to 30 sizes. IE: 'Michelin Pilot Sport Cup' comes in 15 different sizes. Each size will have it's unique product page and description bringing me to my question. Should I use the same description on every size? I do plan on writting unique content for each tire model however i'm not sure if I should do it for every size. After all the tire model description is the same for every size, each size doesn't carry any unique characteristics that I can describe. Thanks in advance!

Woah, my A-grade optimized pages that were on the first page have all vanished outside the top 50 in Google... is this Panda?
Yep as mentioned, I had some pages hit the top ten pretty quickly after ensuring they were A-grade for pretty uncompetitive keywords however, today, all bar 2 have just vanished from the top 50! All were ranking in Google.co.uk and I know we were predicting the Panda update to hit soon but WOW! My client is understandably a little upset, what on earch do I tell him and what should I do about it? Cheers! jT
