SEO issues with Physician and Practice not ranking for their own names

I've inherited an SEO client who's got all kinds of ranking problems. Currently his name Dr. Laddis shows up for his old practice, Saratoga Cardiology Associates (http://saratogacardiology.com/) instead of his current one Cardiology Specialty Services (http://cardiologyspecialtyservices.org/ ) He's also showing up with a G+ for the old practice that's listed as closed. The 2nd listing is for his bio on the hospital page.(<cite class="_Rm">https://saratogahospital.org/doctor/theodoros-laddis-md-facc/ )</cite> Then come the usual Dr directories. His YouTube channel shows up. But his actual website isn't until the middle of the 2nd page.
I'm also having similar issues getting the practice to show up in search (http://cardiologyspecialtyservices.org/ ). As I was coming on board, they also had a name change from Saratoga Cardiology Specialty Services to Cardiology Specialty Services. Their G+ local business page has the custom URL for Saratoga Cardiology Specialty Services but the name on the page is Cardiology Specialty Services.
Their website is actually part of the hospital multisite with a URL redirect.
While the site shows up for "cardiologist Saratoga" their G+ page doesn't show up.(https://plus.google.com/+Saratogacardiologyspecialtyservices/about ).
I've also done on-page SEO and am still in the process of submitting to directories.
Any thoughts on what the hangup is or what I can do to clear up this mess would be appreciated.

Ilya, yes, the G+ business page is verified. Holding off on Laddis' page because the other drs in the practice have pages set up, but I'm not finding any for Laddis--so waiting to hear back if someone at the hospital has set that up already. I wish I could log into one dashboard.. They've had so many people working on this and leaving without creating a central database of work done or logins, etc., over the years that it's a jumbled mess.
Miriam, I've been working through Moz Local to add to directories, update current info and clean up duplicates. Thanks so much for the link to the article!

Hi There!
One thing that may be helpful to you is to put both business through our free Check Listing tool to start to discover duplicate, incomplete and inconsistent listings. Go to moz.com/local/search and enter both the current name and zip of the businesses and also try adding any old names and variants. You need to create a spreadsheet of every inconsistent citation that exists so that you can embark on a citation cleanup campaign, editing old references to reflect new data. Hi Karen,
Have you claimed the Google My Local Business listing for_ Dr. Laddis _or Saratoga Cardiology Associates?
If you go to http://www.google.com/business/ you should be able to log in using client's details and see all Google+ pages associated with the business. You can edit those pages from there and update the listings.
If there are duplicate listings that are no longer required, you can delete them here as well.

Why would a domain rank well in some markets and poorly in others?
We are having an issue with our SERP results. Basically here is what’s happening. We offer training in 47 cities. Our main sales page is [http://example.com/product/city-name]. Whenever we change our location preferences in Google, and that URL format is the landing page for our main 3 keywords, we rank no worse than 15. Most of those are on the first page. However, if that is not the landing page in the SERPs, we rank poorly. Depending on which keyword is used, one of three URLs will ALWAYS appear instead of the desired city-specific landing page, and we rank somewhere between pages 6 and 9. The other landing pages, depending on which keyword is used, are http://example.com (home page), http://example.com/product (without the city), or http://example.com/product/san-francisco (regardless of how close this is). Out of the 141 keywords, there have been between 9-25 keywords that have this issue. Having all 3 of these keywords in the top 10 for each city is the number one priority from our CEO. We have noticed if a page starts moving down the ranks, once it goes past 15, the landing page changes. Anyone have any ideas what could be causing this, or how to fix it? Any insight into this is much appreciated!

Brand Name Pulling Into Search Results Incorrectly
Hi Moz Community! I have a client, Acme Stove Co., whose branded search results are pulling in the wrong name. A Google search for "Acme Stove Co" returns pages with the title "Acme Stove & Video." Really not sure where this "& Video" part is pulling from - there is no mention of videos on the pages that are ranking with these terms, and Google is definitely using a title tag other than the one that we have worked to optimize. It appears to be happening on the homepage and our store location pages. Screen shot of search results is attached. The company definitely does not offer any sort of video services whatsoever, so we're really stumped as to where this may be coming from. Any insight would be greatly appreciated!! 8bNOWeF.png

Can I use a fake email address for Moz Local submission? This is for a medical practice, that has been advised not to publish an email address due to HIPAA concerns.
I work with medical practices, who due to HIPAA do not want to publish an email address in listings. Would it cause a problem if I just submitted a non-working address (like noreply@client.com) that is not set up at all, so any email sent there would just bounce? Obviously, one option is to use a HIPAA compliant email account, but there are other considerations (timeliness of responses related to clinical issues, for example) that make it less than ideal for a many medical practices to publish a public email address of any sort. I'm interested in any options or solutions that could allow me to use Moz Local (which requires an email address) for businesses with HIPAA concerns. Ira
