Keyword stuffing - on page grader count

I used the on page grader for my homepage, www.cprnj.com, and it said the keyword "physical therapy" was used 26 times in the body of the page. But I can't find more than 4.
How exactly is keyword stuffing accounted for, and is there some place that I am not seeing the keywords?

Hi Jason. While it is a best practice to use the keyword in the URL, I think you did overdo it and are running into something that would be considered keyword cannibalization within your own site. While an old blog post, the core principles remain the same here: https://moz.com/blog/how-to-solve-keyword-cannibalization. To get around using 'physical therapy' excessively, you can break down the different types of PT into: treatments, exercises, stretches, and so on. This semantic use of similarly related keywords is a positive signal versus the negative of keyword stuffing. Cheers!

- with regards to the links, from what I've read, a lot of SEO suggestions suggest that you place your most important keyword in your url links, ours being "physical therapy". Did I overdo it a little? I thought it was very basic and descriptive overall, i.e. we do vestibular physical therapy, so the url is /vestibular-physical-therapy
- the moz on page grader says the term was in the body 26 times. I counted 4, and it looks like you counted 4 as well. is there somewhere else I'm not looking?

Thanks for the help! I've been trying to figure out the H1 tag stuff, but I'm using Squarespace and am trying to figure out how to get access to that portion.
Also, with the title tag, unfortunately, I'm not sure how I can shorten the homepage title tag. I feel like I've shortened it as much as possible, basically, most important keyword (physical therapy), location (jersey city, elizabeth), and company name. I've been debating on removing the company name, but it's the first thing that shows up on our Google search result listing. Thoughts?

Ryan is right from a technical view.
But, i would be very hesitant to change the page based on the onpagegrader. My assumption is that google looks at a website and takes the content a customer can see. The fact that the keyword appears in the links seems somewhat high, but you are a into physical therapy! ( the alt text seem varied already)
So unles you have a problem with google i would not focus on that.
Focus on the fact that you should have just 1 H1 Tag ( prefereably not a picture) and the way to long title tag. Fix the / non dash duplicate content problems etc etc..

Hi Jason. Beyond the body text, the term physical therapy shows up heavily in many ways.
- Alt text. 6 times
- Tag Anchor text. 12 times
- As 'physical-therapy' in links. 260 times.
- As 'physical+therapy' in links. 21 times
- Body text. 4 times.
If you look at the code, it's a pretty heavy usage of the term on the home page and throughout the site in on way or another. I'd work on reducing the presence in links and making alt text more descriptive of the image being described than as a box to tick for the keyword location. Cheers!

403 error but page is fine??
Hi, on my report im getting 4xx error. When i look into it it says the error is crital fo4r 403 error on this page https://gaspipes.co.uk/contact-us/ i can get to the page and see it fine but no idea why its showing a 403 error or how to fix it. Keyword Difficulty Tool not working!
Hello everyone, I quite new to the pro service and I am still on my free trial and I have to say that it is perfect! However, I haven't been able to use the keyword difficulty tool for a couple of days as it keeps saying that there is a problem and that I should try again in 20 minutes. I tried several times and 9 out of 10 times I get this message. I have only managed to get a couple of keywords analyzed. Keep in mind that most of the times I am not using English terms but since it worked in the past I would think that this is not the problem. Any thoughts on how to fix this issue? Thanks in advance!

Tracking Keywords
I want to remove keywords to one campaign, but i have several hundred being tracked. Does anyone know if i delete them all, then re-add the ones i want will i loose the ranking history? Thanks

Seomoz crawling filtered pages
Hi, I just checked an seo campaign we started last week, so I opened seomoz to see the crawl diagnostics. Lot's of duplicate content & duplicate titles showing up, but that's because Rogerbot is crawling all of the filtered pages as well. Twitter Page Authority Score?
I've been doing some competitive research in Open Site Explorer and many of our competitors have Twitter accounts very similar to ours. Their Twitter pages are usually one of the pages with linking to their website with the most Page Authority. The incoming links from Twitter are a "no follow" as you would guess. This has been the case for a large number of well ranking sites I have looked at. www.dremed.com also has a Twitter account at: https://twitter.com/#!/DREmed . However, Open Site Explorer does not list the Twitter link as an incoming link at all ( or if it does it has no Page Authority ). The Twitter account page seems very similar in nature to other competing Twitter pages. I'm not sure why it does not ALSO pull a high Page Authority score??? Do you know why this might be? Keyword Canibalization!
I have a page and it's ranking for a keyword. I placed an internal text link with that keyword as an anchor from that page to that page. The pages is linking to itself. Why on-page report card sees that anchor text link as a cannibalization link? Another problem that i cannot resolve about keyword cannibalization is that the same tool sees cannibalizing link for different kw. Ex : I have a page/category about shoes. From that page i link with anchor text links (the name of the anchor is the name of subcategory ) to subcategories which are men shoes, women shoes, kids shoes, etc. Now, why those links are seen as a cannibalization links for the keyword shoes?
