Trying to pinpoint why 1 keyword moved down 100 positions in 2 weeks. Help me speculate?

Hi there,
One of my client's sites, a very large and successful ecommerce website with great SEO performance, has seen a significant drop in rankings in the past 2 weeks. The rankings have begun to somewhat stabilize today, except one particular keyword with a search volume of 74k has gone from 1 to 100.
Here is what has taken place in 2 weeks, sitewide:
- I revised and improved upon title tags and meta descriptions to make them more user-friendly and contain more optimized terms. Following all of Google's best practices, as always. Google still appears to be indexing these changes (has anyone seen an initial drop in rankings while this takes place?)
- The site has seen a very significant increase in 404 errors due to one feature of the site breaking. We got a message about it in Webmaster Tools, and this appears to coincide with when overall rankings dropped. The development team is working quickly to get this resolved.
- As of today, I am seeing the highest page-load time than any other day in 2015.
With regard to the particular page/keyword in question:
- The keyword is no longer "exact match" at the beginning of the title tag, but rather broken up throughout the title tag so the whole title sounds better for users. **Have you found that this type of change is sufficient for a keyword rank to move down ~100 positions?? **(Either way, I have asked the client to revise the title to start with the exact match keyword, once again.)
- Google has indexed the page 2 days ago, but is still displaying the old title tag in search results.
- I have not found any instances of internal or external links to this page being removed.
With all this information, does anyone see anything that seems like it could have reasonably caused such a huge tank in rankings? Is this a blip in time? Is there anything I am not considering? Should I just be patient?

So turns out I just checked again today and my client is back at position 6.
I guess Google was just "re-calibrating?"

Can you pinpoint a specific date the site dropped in ranking? Maybe check for a drop in Google organic traffic in analytics or a loss of impressions etc in Search Console. Or a date the ranking shifted? That's usually the best way to rule out an algo update. I think 11/19/2015 was the date of this last one.

Thanks for your help.
I have a hard time believing it's the result of any of these algorithm updates because this is a very high-quality site with fantastic content. The keyword I am focusing on is a head term in the medical industry with very high volume. We were #1 several weeks ago. The ranking page offers about 750 words of excellent content structured sort of like Wikipedia or Web MD, with helpful internal links and medical sources listed on the bottom.
I did have the client make one or two revisions and update the date associated to the article, as it was 2011, and now it's 2015.
I'm still concerned because today it looks like the page fell even deeper in the index.
Any thoughts?

I don't think any of those factors with the information provided could have had such an impact on the site. Especially not some simple title tag adjustments. It's possible that would happen over time as Google processes the new user signals, but probably not so quickly.
What type of keyword was it? What industry is the eCommerce site in?
Not sure if this came on your radar, but there was an unconfirmed Google update a few weeks ago:

If the changes you see match about November 19th, I'd check out Glenn's suggestions at the bottom of that second article. The site could have been caught in a content quality update.

Unexplained Drop In Ranking and Traffic-HELP!
I operate a real estate web site in New York City (www.nyc-officespace-leader.com). It was hit by Penguin in April 2012, with search volume falling from 6,800 per month in March 2012 to 3,300 by June 2012. After refreshing content and changing the theme, volume recovered to 4,300 per month in October 2013. There was a big improvement in early October 2013, perhaps tied to a Panda update. In November 2013 I hired an SEO company. They are reputable; on MOZ's recommended list. After following all their suggestions (searching and removing duplicate content, disavowing toxic links, improving the site structure to make it easier for Google to index listings, re-writing ten key landing pages, improving the design of the user interface) ranking and traffic started to decline in April of 2014 and crashed in June 2014 after an upgraded design with improved user interface was launched. Search volume is went from 4700 in March to around 3800 in June. However ranking on the keywords that generate conversions has really declined, and clicks from those terms are down at least 65%. My online business is severely compromised after I have spent almost double the anticipated budget to improve ranking and conversion. A few questions: 1. Could a drop in the number of domains lining to our site have led to this decline? About 30 domains that had toxic links to us agreed to remove them. We had another 70 domains disavowed in late April. We only have 78 domains pointing to our domain now, far less than before (see attached AHREFs image). It seems there is a correlation in the timeline between the number of domains pointing to us and ranking performance. The number of domains pointing to us has never been this low. Could this be causing the drop? My SEO firm believes that the quality of these links are very low and the fact that many are gone is in fact a plus. 2. The number of indexed pages has jumped to 851 from 675 in early June (see attached image from Google Webmaster tools), right after a site upgrade. The number of pages in the site map is around 650. Could the indexation of the extra 175 page somehow have diluted the quality of the site in Google's eyes? We have filed removal request for these pages in Mid June and again last week with Google but they still appear. In 2013 we also launched an upgrade and Google indexed an extra 500 pages (canonical tags were not set up correctly) and search volume and ranking collapsed. Oddly enough when the number of pages indexed by Google fell, ranking improved. I wonder if something similar has occurred. 3. May 2014 Panda update. Many of our URLs are product URLs of listings. They have less than 100 words. Could Google suddenly be penalizing us for that? It is very difficult to write descriptions of hundreds of words for products that change quickly. I would think the Google takes this into account. If someone could present some insight into this issue I would be very, very grateful. I have spent over $25,000 on SEO reports, wireframe design and coding and now find myself in a worse position than when I started. My SEO provider is now requesting that I purchase even more reports for several thousand dollars and I can't afford it, nor can I justify it after such poor results. I wish they would take it upon themselves to identify what went wrong. In any case, if anyone has any suggestions I would really appreciate it. I am very suspicious that this drop started in earnest at the time of link removal and the disavow and accelerated at the time of the launch of the upgrade. Subpage ranking for homepage keyword
Hi all, May seem like a simple scenario and I might be missing something, but my subpage seems to be ranking for my main homepage keyword. The subpage PR is 28 and my domain authority is 17, how can I get my main home page to rank instead of the sub page (product page)? I want to stay away from exact match anchor text links, any suggestions?

Help my site it's not being indexed
Hello... We have a client, that had arround 17K visits a month... Last september he hired a company to do a redesign of his website....They needed to create a copy of the site on a different subdomain on another root domain... so I told them to block that content in order to not affect my production site, cause it was going to be an exact replica of the content but different design.... The developmet team did it wrong and blocked the production site (using robots.txt), so my site lost all it's organica traffic, which was 85-90% of the total traffic and now only get a couple of hundreds visits a month... First I thought we had been somehow penalized, however when I the other site recieving new traffic and being indexed i realized so I switched the robots.txt and created 301 redirect from the subdomain to the production site. After resending sitemaps, links to google+ and many things I can't get google to reindex my site.... when i do a site:domain.com search in google I only get 3 results. Its been now almost 2 month and honestly dont know what to do.... Any help would be greatly appreciated Thanks Dan

Owning all of the top 10 positions for one keyword
Hello, I would like to know if it is considered bad by Google to rank every single spot in the top 10 with the same keyword? All websites conver the same topic which this keyword, but have unique content. Those websites dont link to one another. To be clear, I am not asking if it is a good idea because it will be harder to rank, find links etc. or because the last positions don't bring enough traffic. I really just would like to know if it against Google guidelines? Any thoughts or sources are greatly appreciated.

Please help on this penalized site!
OK, this is slowly frying my brain and would like some clarification from someone in the know, we have posted multiple reconsideration requests the regular "site violates googles quality guidelines" .."look for unnatural links etc" email back in March 2012, I came aboard the business in August 2012 to overcome bad SEO companies work. So far i have filled several disavow requests by domain and cleared over 90% of our backlink profile which where all directory, multiple forum spam links etc from WMT, OSE and Ahrefs and compiled this to the disavow tool, as well as sending a google docs shared file in our reconsideration request of all the links we have been able to remove and the disavow tool, since most where built in 2009/2010 a lot where impossible to remove. We managed to shift about 12 - 15% of our backlink profile by working very very hard too remove them. The only links that where left where quality links and forum posts created by genuine users and relevant non spam links As well as this we now have a high quality link profile which has also counteracted a lot of the bad "seo" work done by these previous companies, i have explained this fully in our reconsideration request as well as a massive apology on behalf of the work those companies did, and we are STILL getting generic "site violates" messages, so far we have spent in excess of 150 hours to get this penalty removed and so far Google hasn't even batted an eyelid. We have worked SO hard to combat this issue it almost feels almost very personal, if Google read the reconsideration request they would see how much work we have done too remove this issue.
