404 crawl errors with all url+domain

We have 187 crawl 404 errors. All urls on web make a 404 error that this
http://www.domain.com/[.....]l/www.domain.com
all errors added to the url, the url domain
I put an example

Look in your footer at the contact information, right below the information email. There's an http:// missing in front of www.gestoriabarcelona.com.

referreing URL is the same URL, but on font code i don't see that URL

No, website has crawled 5 days ago, and errors still there.

If you download the CSV, there's a column in there for referring URL. It looks like some URLs may have gotten an improper link.

Dates in URL's
I have an issue of duplicate content errors and duplicate page titles which is penalising my site. This has arisen because a number of URLs are suffixed by date(s) and have been spidered . In principle I do not want any url with a suffixed date to be spidered. Eg:- www.carbisbayholidays.co.uk/carbis-bay/houses-in-carbis-bay/seaspray.htm/06_07_13/13_07_13 http://www.carbisbayholidays.co.uk/carbis-bay/houses-in-carbis-bay/seaspray.htm/20_07_13/27_07_13 Only this URL should be spidered:- http://www.carbisbayholidays.co.uk/carbis-bay/houses-in-carbis-bay/seaspray.htm I have over 10,000 of these duplicates and firstly wish to remove them on block from Google ( not one by one ) and secondly wish to amend my robots.txt file so the URL's are not spidered. I do not know the format for either. Can anyone help please.

Why do I suddenly have so many more page errors?
I've been working diligently to remove page errors that appear after each scan of my website. I was down to 3 or 4 errors after the last crawl, now this week I'm getting 80 plus. I haven't changed anything since last week (page titles, etc.) so what's up?

Importance of URL Structure
We are trying to restructure our onpage SEO and want to make sure we have our URLs correct. The problem is we did the URLs incorrectly in the first place and the ones we currently have are several years olds. We have some URLs such as: http://www.firebrandtraining.co.uk/courses/management/prince2.asp and

http://www.firebrandtraining.co.uk/courses/cisco/ccna_2007.asp which are not ideal but user experience aside does it make sense for us to change the URLs and use 301 redirects to the new ones or is the damage done to our natural rankings simply not worth making the change? I have read different articles saying different things, some say that URL structure has little weight (if any weight at all) on rankings while other people seem to say it is quite important. In addition we have heard that changing the URLs with a 301 redirect will cause a large drop in ranking which will take months to recover from and contrarily that 301s are now considered "ok" by Google and we shouldn't see too much change at all in our rankings. Include the company/domain name in page titles and urls?
I know this isn't something that I would use site-wide but I'm wondering if it helps or hurts me to use my company name (also my domain name) in pages below the homepage. As an example, let's say I'm Home Depot. In the category pages off the homepage should I use Page names and urls like Home and Garden Supplies or Home and Garden Supplies at Home Depot? Or does it hurt me to reuse my company/domain name on multiple pages?

Why has only one page been crawled?
I went over the list of possible reasons, but I don't think any of them apply to the site in question: sarahhueniken.com. Any other reasons only one page would be crawled?
