Spike in 4xx Client Error After Theme Change

There is a huge Spike in 4xx Client Error after new Wordpress Theme Change.
Report shows URL that doesn't exist in the Referral Page.
Missing URL (404) - http://www.happyschoolsblog.com/gre-test-dates-2011/happyschools/
Referral - http://www.happyschoolsblog.com/gre-test-dates-2011/
Likewise there are over 4000 errors (4xx) with happyschools appended to the URL. I'm not sure how to fix the those errors. Thanks.

Added absolute path to Twitter Handle. This should remove some 4000 or so 4xx errors.

Why do I see a duplicate content errors when rel="canonical" tag is present
I was reviewing my first Moz crawler report and noticed the crawler returned a bunch of duplicate page content errors. The recommendations to correct this issue are to either put a 301 redirect on the duplicate URL or use the rel="canonical" tag so Google knows which URL I view as the most important and the one that should appear in the search results. However, after poking around the source code I noticed all of the pages that are returning duplicate content in the eyes of the Moz crawler already have the rel="canonical" tag. Does the Moz crawler simply not catch whether that tag is being used? If I have that tag in place, is there anything else I need to do in order to get that error to stop showing up in the Moz crawler report?

Crawl Diagnostics Error Spike
With the last crawl update to one of my sites there was a huge spike in errors reported. The errors jumped by 16,659 -- majority of which are under the duplicate title and duplicate content category. When I look at the specific issues it seems that the crawler is crawling a ton of blank pages on the sites blog through pagination. The odd thing is that the site has not been updated in a while and prior to this crawl on Jun 4th there were no reports of these blank pages. Is this something that can be an error on the crawler side of things? Any suggestions on next steps would be greatly appreciated. I'm adding an image of the error spike Xovep.jpg?1 Xovep.jpg?1

Crawl Diagnostics shows two title and meta tag errors but they are false positives.
I got one hit each on "Missing Meta Description Tag" and "Title Missing or Empty" but in the source of my page they are clearly there: <title>Protein Powder | Compare and Get the Best Prices</title> <meta name="keywords" content="protein powder, whey protein, protein supplement, whey protein isolate, hydrolyzed whey" /> I understand there are conventions which may or may not be followed by Drupal (I read an earlier question where ordering and W3C conventions were suggested) but i'm not sure how to fix them given Drupal will just overwrite any hand editing the next time something is built and importantly, I can't get the crawl to work on cue - it works on the automatic once a week crawl in the main campaign summary but every time I've specifically used the Crawl Test tool it gives me a "There was an error submitting your request to the crawler. Please try again later" so I can't really test any changes. Given Google seems to be recognising the title tag - ie showing it in the results - Do I put this down as seomoz just not working?
