404 : Errors in crawl report - all pages are listed with index.html on a WordPress site

Hi Mozers,
I have recently submitted a website using moz, which has pulled up a second version of every page on the WordPress site as a 404 error with index.html at the end of the URL.
e.g
Live page URL - http://www.autostemtechnology.com/applications/civil-blasting/
Report page URL - http://www.autostemtechnology.com/applications/civil-blasting/index.html
The permalink structure is set as /%postname%/
For some reason the report has listed every page with index.html at the end of the page URL. I have tried a number of redirects in the .htaccess file but doesn't seem to work.
Any suggestions will be strongly appreciated.

Spot on with your answer!

Its your logo thats causing the issue.
Notice that its supposed to link back to the home page, instead its linking to the page your on and its including /index.html at the end of the URL.
Not sure how the header image is set up, but have a look at that, it will fix your 404 errors.

Low KDS but high DA for all page 1 sites
Hi there A KDS report question... My client has a keyword with a low KDS (garden hose - 28). Intuitively this seemed too low, and when I checked what ranks, it is all high quality brand sites with high DAs. Admittedly the specific pages ranking have low PAs, but I can't see my client ever being able to compete with these big boys right now. How come the KDS is so low? This seems just wrong as the metric implies it is a good keyword to target - I know there are always lots of other things to check, but this does make me less confident with KDS reporting Anyone got any thoughts on this? 18 404 errors on pages that are actually fine.
Hi, I just used the compain tool to look for errors on my site and it appears that seomoz crawler finds 18 404 errors on pages that are fine in my good. I do proceed with a URL rewritting on those pages, but navigation is fine. Not all pages are being crawled
I am set up on the PRO plan, I was under the impression that it would crawl up to 10,000 pages. My site has just over 200 pages, but whenever I am crawled it only crawls 121 pages. Is this normal? It's hard to know how reliable my data is because a significant amount of pages are missing.

Why does Crawl Diagnostics report this as duplicate content?
Hi guys, we've been addressing a duplicate content problem on our site over the past few weeks. Lately, we've implemented rel canonical tags in various parts of our ecommerce store, over time, and observing the effects by both tracking changes in SEOMoz and Websmater tools. Although our duplicate content errors are definitely decreasing, I can't help but wonder why some URLs are still being flagged with duplicate content by our SEOmoz crawler. Here's an example, taken directly from our Crawl Diagnostics Report: URL with 4 Duplicate Content errors:

Is there any way to view crawl errors historically?
One of the website's we monitor have been getting high duplicate page titles, as we work through the pages, we see changes and the number of duplicate page titles are decreasing. However, lately, it went up again and the duplicate page titles have increased. I wanted to ask if there's any way to view the new errors and the old errors separately or sorted in a way that can help me identify why we are getting new page crawl errors. Any advice would be great. Thanks!
