SERP Question - Site showing up for national term over local term recently

This has been happening to me with a couple of clients recently and I wanted to kick it out to the community and see if anyone else has experienced it and might be able to shed some light on why.
(Disclaimer: Both clients are in the elective healthcare space)
Scenario: Client's site is optimized for a fairly competitive "procedural keyword + location" phrase. Historically, the site had been ranking on the first page for a while until it suddenly dropped off for that query. At the same time, the page now ranks on the first page for just the procedural term, without the location modifier (obviously much more competitive than with the location modifier). Searches on Google were set to the city in which the client was located.Not that I'm complaining, but this seems a little weird to me. Anyone have a similar situation? If so, any theories about what might have caused it?
TL;DR - Site ranked on 1st page for "keyword + location modifier" historically, now ranking on 1st page for "keyword" only and not found with "keyword + location modifier"

I see what you're saying, but the client isn't in my city - I'm searching as if I was in the same location as them so I want Google to display as if I'm searching in that area.
What's weird to me is that Google would be displaying the client (organic and local pack) for a head term but not for the 'head term + city' in a search presumably performed in the same city.
Conceptually I get it, perhaps the algo is getting better at searcher intent without the need for adding the geo-location. What I can't wrap my mind around is why the site is showing up in a SERP for a national term and at the same time not being found anywhere for the head term + city for which it was optimized. Redundancy aside, you could assume if it was getting listed for the national term, it would also be found for the longer-tail query.

when you just search for the head term and google knows that you live San Luis Obispo, CA it will show you search results influenced locally.
go incognito ... change the location from San Luis Obispo, CA to United states and do the same search
then again to the search of keyword + city

Yes, same city modifier as the city where the search was performed. My client was not showing up in either the local pack or organic listing when keyword + city was searched.
BUT there were (and still are) instances where the client was showing up in BOTH the local pack and organic listings for just the keyword search.

is the city modifier in the same city you are doing the search ?
when you mentioned that when you do a search keyword + city ... the company was showing in the organic or local pack ?

Less relevant/not optimized competitor sites ranking higher in SERPs?
Has anyone else noticed their rank positions falling to competitor sites that aren't optimized and are less relevant? I've noticed that we've lost some rankings or have dropped over the past few weeks and the competitor pages that have replaced us haven't been optimized, aren't as relevant, and it doesn't look like there has been any updates (looking through archived versions). For example, their main "shoes" gallery is ranking for more specific shoe types, like "sandals", and "sandals" isn't even mentioned in their metadata and they have no on-page copy. Their DA is slightly higher, but our sites have a denser link profile (although, yes, I do need to go through and see what kind of links, exactly, we've gained). Has anyone else seen this happen recently, or have any ideas of why or what we could do to get our rank positions back? My main initiatives have been to create and implement fresh on-page copy, metadata, and manage 404s/301 redirects, but I'm thinking this issue is beyond a quick copywriting tweak.

Category Containing a Product searched shows up higher in google then the product page itself?
Hello Moz Wizards, We have recently launched a new eCommerce website www.memoky.com and think we did a pretty good job with the markup structure for feeding the hungry google bot all information available about a the products. However google doesn't like us very much : ( It seems every time you google a product that we carry; the category pages that contain that product will show up, but the product page itself does not. Below are two examples, however this seems to be site-wide which makes me feel like there is an underlying issue that we are missing. Examples

Pages fluctuating +/- 70 positions in Google SERPs?
I've got some pages that appear somewhere around #25 in Google. Every now and then, it just goes away from the top 100 results for a few days (even up to a week) and then it comes back. I've got other pages that rank around #8 which falls down to about #75 for a while and then it comes back. But while a page may be gone from the top 100 results in the US, it still ranks at about the same place everywhere else in the world (+/- 10 positions). I've seen this happen in the past but never it happened so often. What gives?!?

Can any one help me to increase the site traffic? What are the techniques i need to follow to increase site visits? Also one of my sites recently got disappeared from google. I have checked all the pages listed in google for my site's major keywords. I didnt find the site anywhere. Can u hep me why this condition wll happen and what to do to overcome such issues?

My content is quite strong within my niche, so I ranked well, but last month my rankings plummeted. On closer examination and scrutiny I discovered my anchor text needed updating. Has anyone else seen this happening in the last four weeks?

Google removing pages from Index for Panda effected sites?
We have several clients that we took over from other SEO firms in the last 6 months. We are seeing an odd trend. Links are disappearing from the reports. Not just the SEOmoz reports, but all the back link reports we use. Also... sites that pre Panda would show up as a citation or link, have not been showing up. Many are these are not Indexed, and are on large common Y.P or other type sites. Any one think Google is removing pages from the Index on sites based on Panda. Yours in all curiosity. PS ( we are not large enough to produce quantity data on this.)
