Page Title "Untitled Document" with mozbar

Hi,
I'm working on a big news website (in tourism) and, when checking my homepage with mozbar, I often get:
"My Title • Untitled Document" - 69 characters
With no apparent reason, If I refresh the page or simply visit it later, it could randomly switch to the "normal" title:
"My Title" - 44 characters
It looks like completely random to me, like something more is loaded in the first case.
I've checked my source code of course, and <title>tag seems to be ok with no other <title> even when I get the "untitled document".</p> <p>I have no idea about that. I don't know if that can be a useful info, but I was trying to understand why the majority of my main keywords searches are only showing other pages (I can rarely find my homepage) even though my home looks correctly indexes and shows as first result for every site: research.<br /><br />Hope everything's is clear....any idea or suggestions?<br /><br />Thank you in advance.</p> <p> </p></title>

I managed to auto-resolve that. I tried to delete the question but looks like I can't.
So, if that can be useful to someone else, I had an iframe in the page with a <title>Untitled document</title> (!) which was causing that.

ERROR: Too Many on-page links!
User wise, my product menu @LEDSupply.com is user friendly, but I'm concerned that it might be seen by crawlers as bad because of the TOO MANY ON-PAGE LINKS error I am getting in my moz crawl report. Is it really counting all the links in every drop-down menu? If so, is there are resource on how to fix it????

How do I find out which pages are being indexed on my site and which are not?
Hi, I doing my first technical audit on my site. I am learning how to do an audit as i go and am a lost. I know some page won't be indexed but how do I: 1. Check the site for all pages, both indexed and not indexed 2. Run a report to show indexed pages only (i am presuming i can do this via screaming Frog or webmaster tool) 3. I can do a comparison between the two list and work out which pages are not being indexed. I'll then need to figure out way. I'll cross this bridge once i get to it Thanks Ben

Duplicate Page Title - although there are differences
Hello, I get duplicate page titles errors on pages in which there are little differences. For example: C++ Online Test for Seniors C# Online Test for Seniors I assume that from some reason the ++ and the # are removed when SEOMoz crawler checks for duplicate page titles. As you may know C# and C++ means two different programming languages. Should I do something about it or is it a bug in the crawler?

Rel="canonical" tag is implemented in my product pages, but still getting canoncal error for products in Moz. What is the problem? me or MOZ?
I have included the rel="canonical" tag in all my product pages, but still getting canonical error in MOZ reports for more than 6 month ! I would like to know if my code is wrong or the MOZ report system is not working properly. Getting odd results with MOZbar. (Some pages are 0,0,0)
I'm trying to review the Domain aurhotiry, Page Authority, and MozRank & Moztrust for some news websites and I found it odd that may sites will have excellent DA,MT,MR & PA on most of the pages but then when I view one of their blog posts the PA,MR & MT are 0. Here's are two examples Site

On Page Analysis and Grading
I am new here and happy to be! My site is an ecommerce site with hundreds of products. I have set up campaigns to track specific products. For the on page analysis where SEOMOZ gives you a grade I have 2 urls showing. But 1 of the urls is getting an A, and 1 is getting a F. But they are the same url and obviously go to the same page. Any help would be appreciated!
