How do I create multiple page URLs that are optimized for location and keywords that may be overlapping or the same?

Hi guys,
I am attempting to create unique URLs for several different pages on a website. Let's say hypothetically that this is a website for a chain of Ice Cream Shops in Missouri. Let's say they have 15 locations in Springfield, Missouri. I would ideally like to optimize our Ice Cream Shop's in Springfield, Missouri with the main keyword (ice cream) but also the geo-specific location (Springfield), but we obviously can't have duplicate URLs for these 15 locations. We also have several secondary keywords, think things like: frozen yogurt or waffle cone that we can also use, although it would most likely be more powerful if we use the primary keyword.
Any suggestions for how to go about doing this most effectively?

How do your customers differentiate between stores? Use whatever language they use. So for example, you could name the pages sunshine-rd-ice-cream or a rte-13-ice-cream. That's how I'd do it.
I'd also customize title and description tags, and include a keyword-named photo of each shop (with corresponding alt tags) and an embedded map showing it's location. That's another way to ensure the content of each page is unique b/c that will need to be the case. You could talk about the most popular flavors per location, or the busiest times (in case site visitors are trying to avoid crowds or alternatively, seeking them out).

We have about 30 pages of content that we are hiding from the website as these articles had some issues. If these pages ranked well, would you recommend that the new content is written within these pages? Meaning, we would replace the content that's in those pages with the same topic and keywords. Changing posts that target same keyword to target SIMILAR keywords
Hello, I'm new to SEO and until the last year I was not aware that creating content that target same keywords would make Google choose only one to rank better. Can I leave the best page (highest ranked one) as it is and change the rest that target the same keyword to target similar keywords ? Just a simple example: I have two pages targeting "best supplements to muscle gain" Leave the best one targeting that keyword and now I change the content of the other to target "best supplements for muscle size" This is the smartest move or I'm missing something here and will make things worse ? Ex: maybe some of tthose pages with same keyword arent ranking for the keyword I want but are ranking for another one that I dont know. I'm really insecure about making changes and end up getting worse results.. Thank you (sorry for my bad english)

Title tag and URL Optimization
Hello guys, Should the URL reflect the structure of the title of a webpage? This is the old title with the Url: 20mm O/D Black Polypropylene LSZH Flexible Conduit 100m Coil /Product/20mm-o-d-black-polypropylene-lszh-conduit-100m-coil/1352 I changed the keyword position and it looks like this: 20mm Flexible Conduit | O/D Black Polypropylene LSZH | 100m I kept the same Url for now, should I change that too? Are there any SEO benefits changing the default home page filename (index.htm) to a keyword rich filename
II'm a newbie. I have a website using the default home page filename: index.htm. I have total control over the web server. I was wondering whether I can get any SEO improvements for my main keyword if I change the default filename with a filename that contains the main keyword, like our-main-product.htm (doing the 301 redirect and changing the server search order, of course)?

How important is it to include the target keyword phrase in the page URL?
If I want to target a keyword phrase to a particular phrase, but do not want to change the URL of that page, will that negatively impact my rankings? I am also wondering if I can get around it by creating a new, short URL that 301 redirects to the original URL. Would that be as effective as including the keyword in the original URL?

My homepage no longer ranks for a keyword, instead a page from the blog now appears in the results
Hello, Our site used to rank for a specific keyword - "eco products", with a link going to the homepage. However, in the last few weeks, we've noticed that the home page no longer ranks for this keyword, and instead the second result on the page is a link to a catagory page on our blog.
