Number of pages crawled = 1; Why?

Since November, we've been trying to figure out why, when I select Crawl Diagnostics, my number of pages crawled is only 1.
In mid-november, we changed our URL. That is, we went from www.example.com/home-page/ to www.example.com/new-home-page/. My first assumption was that I needed to re-create my Moz profile. That didn't fix it. The only crawl error we get is the no rel="cannonical" found -- but it's there. We find it on every page, including the home page. Our content shows up in search. Moz bar shows us info for every page. I just don't know what else to check. Everything else in my dashboard seems to look as expected.
Specifically, I've turned to Crawl Diagnostics to find 4XX errors on our site. Typically we find one or two per week. Sometimes 0. Sometimes 4 or more. But it's been 0 since November. I highly doubt we've arrived at perfection.
Any thoughts?

Unsolved 403 crawl error
Hi, Moz( Also reported by GSC)have reported 403 crawl error on some of my pages. The pages are actually working fine when loaded and no visible issue at all. My web developer told me that some times error issues are reported on a working pages and there is nothing to worry about.

Unsolved Almost every new page become Discovered - currently not indexed
Almost every new page that I create becomes Discovered - currently not indexed. It started a couple of months ago, before that all pages were indexed within a couple of weeks. Now there are pages that have not been indexed since the beginning of September. From a technical point of view, the pages are fine and acceptable for a Google bot. The pages are in the sitemap and have content. Basically, these are texts of 1000+ or 2000+ words. I've tried adding new content to pages and even transferring content to a new page with a different url. But in this way, I managed to index only a couple of pages. Has anyone encountered a similar problem?

My duplicate pages are mostly Tag pages...what are best practices?
As I'm looking through my Moz Pro reports on Pages with Duplicate Content, almost all the results are from the automatically created "tag" pages from my blog. I.e., takeflyte.com/flyte/tags/kickapps Should I worry about this? Does it have a negative impact in my search visibility? Should I be using canonical tags on these pages (and if so, pointing them where if there's multiple pages that use the same tags?) How would you recommend handling this issue?

Issues with Page Grader Tool and competitor KW Rankings
I'm having two very frustrating issues with Moz: The page grader tool is not 'connecting' the correct keyword with the correct URL. For instance, if I want to rank for Dallas Motorcycle Accident Lawyer and create page of optimized content at Dallas.Zdfirm.com/Motorcycle-Accident, Moz doesn't grade THAT page. It grades Dallas.ZDFirm.com which to no surprise rates as an F. This is happening with both web sites on this account so much that the page grader tool is not delivering any value. My competitors keywords are not tracking properly. They are showing for "not in the top 50" for keywords that they are on the first page for. Again, this is for both domains on this account. Duplicate Page Title
I am seeing pages show up in my campaign Errors report for Duplicate Page Title. One for example states that this page: https://www.inprocorp.com/AboutInPro/AboutUs/HistoryTimeline/tabid/316/Default.aspx has a duplicate page title as this page: https://www.inprocorp.com/AboutInPro/HistoryTimeline/tabid/316/Default.aspx I have a redirect setup so that if you access page #2, you are redirected to page #1. So how do I get errors like this to drop off my report? Have I not fixed the problem sufficiently for the crawler? Thanks, John

Duplicate Page and Title Content
We applied the tag to our website but Moz still picked up an error in Duplicate Page Content and Duplicate Title for http://www.corpsyn.com/ and http://www.corpsyn.com/index.html after the crawl was completed yesterday. From the forum last week, we tried using the rel=”canonical” tag to tell the Moz crawler that the two pages are the same on purpose but it didn't work.
