I can't replicate this in live SERPs, but Matthew's explanation seems to match the evidence. It does appear that Google is doing some kind of implicit canonicalization. It's rare, but it does happen.
You could try giving the PDF a unique title in its own meta information (the document doesn't seem to have any). It's more likely to look like a duplicate if that's undefined.
The next step would be to recreate the PDF slightly and make it unique, but honestly, each step is going to get to be progressively more work. It really depends whether this is just an anomaly or it's causing you actual issues as a business.

I would say this is a duplicate content issue.
Identical versions of the PDF are found on both the sweetwater.com domain and also americanmusical.com:

Hmm.... This is wacky.
It looks like although it says "Sweetwater" in the Title Tag, the pdf file is still being hosted on your URL (as well as the actual rebate form having American Music listed as the return address).
Maybe perhaps whoever added this rebate form to your website "borrowed" it from Sweetwater? And by that I mean, copied over the pdf file, changed the address, and copied everything else (Title Tag included), but forgot to take off the Sweetwater brand name?
Either way, it's on your own URL and it's a rebate form that includes your own address. Just get into your site editor and change the title tag and you should be fine.

Url shows up in "Inurl' but not when using time parameters
Hey everybody, I have been testing the Inurl: feature of Google to try and gauge how long ago Google indexed our page. SO, this brings my question. If we run inurl:https://mysite.com all of our domains show up. If we run inurl:https://mysite.com/specialpage the domain shows up as being indexed If I use the "&as_qdr=y15" string to the URL, https://mysite.com/specialpage does not show up. Does anybody have any experience with this? Also on the same note when I look at how many pages Google has indexed it is about half of the pages we see on our backend/sitemap. Any thoughts would be appreciated. TY!

Good CTAs for Meta-Descriptions: Direct, Indirect, Narrow, Broad?
It is no secret that good meta descriptions should be written to incite the searcher to click on the result without misleading them. Time and again I read that there are measurable effects by including "strong" CTAs (calls to action). What constitutes a call to action seems by some to be taken really narrow (i.e. "Click here to learn more!" - a very specific action that is spelled out) and by others rather broadly ("... Offer available till December 31" - only implicit, the action [buying/securing] not even mentioned). I now wondered: Many "guides" still recommend rather blunt calls like "Click here", "Read more", "Discover how". Personally I find those really unattractive and often a waste of space. However, I am not the benchmark and favour the informational side perhaps a little too strongly. Do those direct but general CTAs really work well in every case* or should one be more elaborate/indirect? I am looking forward of hearing your experience/opinion! Should we add our company's name in page title tag or not?
We have been adding our company (Townscript) name in all the page titles. For example, in an event page of Lucknow Conclave: www.townscript.com/lucknowconclave the page title is Lucknow Conclave | Alexis Society | Townscript I read somewhere that it's not necessary to put your company's name in the title tag. How to view all 'followed internal links' on a page
I am trying to view all the followed internal links on a few pages of my website. The MOZ toolbar just gives me the total number of internal followed links. What is the best way to actually see all the internal links that are followed by the google bot from any particular page? Thanks in advance.

Can't find Text-Only Cached version in Google search
Hi, I'm trying to view the text-only of a webpage to run a SEO audit, however Google does not give me this option. When i click the two arrows that appear to the right of a search result, the only option I get is Cached. Is there a reason this might be happening? I've tried clearing my cookies, signing out of Google, and anything else I could find on my troll of the internet. I also tried text, only please! however whether this works or not is debatable considering it shows me actual pictures on the site. Any ideas, or maybe another add-on that will work?

Excessive Internal Linking...But it's a product page. What to do?
