I can't replicate this in live SERPs, but Matthew's explanation seems to match the evidence. It does appear that Google is doing some kind of implicit canonicalization. It's rare, but it does happen.
You could try giving the PDF a unique title in its own meta information (the document doesn't seem to have any). It's more likely to look like a duplicate if that's undefined.
The next step would be to recreate the PDF slightly and make it unique, but honestly, each step is going to get to be progressively more work. It really depends whether this is just an anomaly or it's causing you actual issues as a business.

I would say this is a duplicate content issue.
Identical versions of the PDF are found on both the sweetwater.com domain and also americanmusical.com:

Hmm.... This is wacky.
It looks like although it says "Sweetwater" in the Title Tag, the pdf file is still being hosted on your URL (as well as the actual rebate form having American Music listed as the return address).
Maybe perhaps whoever added this rebate form to your website "borrowed" it from Sweetwater? And by that I mean, copied over the pdf file, changed the address, and copied everything else (Title Tag included), but forgot to take off the Sweetwater brand name?
Either way, it's on your own URL and it's a rebate form that includes your own address. Just get into your site editor and change the title tag and you should be fine.

Hi, I originally asked about pros (or lack of them) when it comes to using meta keywords tags here https://moz.com/community/q/importance-or-lack-of-meta-keywords-tags-and-tags-in-drupal. Keyword Appearing on Home Page - Moz Page Grader
Hi Today I entered www.partydomain.co.uk through the Moz Page Grader and found that the Home Page is Ranked B. I noticed that an Area we could improve on is the amount of times we are using our main keyword "Fancy Dress" on the home page. Please can you take a look at www.partydomain.co.uk and scroll to the bottom of the page were the tabs are containing losts of content. I am thinking about removing all of thoose Tabs. Our Competitors dont have any content as such on the home page and are ranking higher than Party Domain for "fancy dress" What do you think ? remove all the tabs to be like the others that rank better? Crawl erros I don't understand
Hi all, after my website is crawled SEMOZ has alerted me about some errors (28 exactly) with the same problem: http://piensapiensa.com/servicios/talleres-para-docentes/taller-sobre-como-motivar-a-los-alumnos/Piensa_Piensa 404 : Error At the end of the URL you can see "Piensa_Piensa" which I haven't added at all. Regarding the META KEYWORDS tag
Two questions regarding the META KEYWORDS tag: Does Google actually penalize you for using the KEYWORD META tag, or does it just ignore it? On a related matter, I've noticed that when I use the On-Page Optimization Tool, it indicates that my header includes the KEYWORD tag, although I've taken great pains to remove keywords from pages that had been optimized by a previous SEO. I'm using Magento - Could it be a legacy issue? Would that make a difference?

Using a more relevant brand title for blog
I'm a newbee here so I appologize in advance for asking a question that might already be aswered ( i looked I promise). The question is this, I've been fiddling with the title tags and came upon the need to make a decision about separating our blog brand to be more specific to it's content. We're a moving company, our primary website talks about services and is branded with our name (%page_name% | 2 Brothers Moving & Delivery Portland Oregon), our blog is a work in progress "Moving Guide" (%post_title% | Portland Moving Guide). How to handle Meta Tags on Pagination... page 2,3,4....
Seems that SEOMoz reports are considering my paginated pages as duplicate Meta Tags. For example, I have a product catalog with 5 paginated pages. Obviously the content on each page is unique and the URL ends in =4, =5 for the page number, but the Title and Description are the same for all the pages. Any suggestions on how to handle this? The pages other than page 1 are not indexed, so it should not be a big deal. But wondering if I should programatically ad the page number to the additional pages to show a difference?

Value of PDF's in SEO
I have a client who has a lot of information in PDF form. They think they should move some of it over into HTML pages so it indexes better. Is there a benefit to converting these PDF's into HTML pages? It seems to me that HTML pages would be good, IF they are relevant pages that could be used online.
