What are we doing wrong?

So, we run seo tests via moz, gtmetrix and other sites. On many keywords (NSFW) we appear around 3-7 on the first page. Our biggest issue is that we are following all the rules moz etc say and we don't move but, but the teo tops sites fail on almost everything we are told to do..... Why? It confuses us. What are we doing wrong?

Ranking #1 in Bing & DuckDuckGo, not at all for Google - where am I going wrong?
According to the Moz rank checking tool, my blog ranks in the top 3 for my name "James Crowley" on Bing, Yahoo (both in the US and UK), and also DuckDuckGo (though Moz can't tell me that). And yet doesn't rank anywhere for Google. I don't have any penalties, and for other keywords it appears fine on Google. Does this seem strange to you? Am I going wrong somewhere? Google Adding Wrong Location to Title Tag on Multi-Branch Business Homepage
We're a business with 5 separate locations across 5 cities in Upstate NY. While doing some visual ad previews in the adwords interface I noticed that Google is altering my title tag and adding the word "Rochester" to the end of it, cutting short my designated title tag. Rochester is the location of our headquarters so not a big deal for 1/5th of our customers. But to my dismay, the same thing is happening when searching from the geo locations of my other branches. So when searching for my business in Buffalo (we have a physical address in Buffalo), the title tag in the results still says our company name and "Rochester". This of course is likely leading to confusion and actively harming our organic CTR in our branch locations. This is happening in all of the remaining 4 branch locations. I'm at a loss, I tried lengthening the title tag but it still gets cut off. The term Rochester appears (as do the other branch locations) in my meta description for the homepage as well as in the text of the page itself. I haven't gone so far as to remove that yet and hopefully don't have to. Does anyone have any ideas? Thank you in advance!

Wrong meta descriptions showing in the SERPS
We recently launched a new site on https, and I'm seeing a few errors in the SERPS with our meta descriptions as our pages are starting to get indexed. We have the correct meta data in our code but it's being output in Google differently. Google is indexing the wrong page
Hello, I have a site I am optimizing and I cant seem to get a particular listing onto the first page due to the fact google is indexing the wrong page. I have the following scenario. I have a client with multiple locations. To target the locations I set them up with URLs like this /<cityname>-wedding-planner.</cityname> The home page / is optimized for their port saint lucie location. the page /palm-city-wedding-planner is optimized for the palm city location. the page /stuart-wedding-planner is optimized for the stuart location. Google picks up the first two and indexes them properly, BUT the stuart location page doesnt get picked up at all, instead google lists / which is not optimized at all for stuart. How do I "let google know" to index the stuart landing page for the "stuart wedding planner" term? MOZ also shows the / page as being indexed for the stuart wedding planner term as well but I assume this is just a result of what its finding when it performs its searches.

The Wrong Page Is Still The Only One Ranking
For some reason, the search term "Tampa Personal Injury Attorney," shows this page of our's http://www.kempruge.com/personal-injury/ on the second page, but omits this page http://www.kempruge.com/tampa-personal-injury-attorney/ (the correct one). The correct one only shows up in omitted results. I have posted this question before. I made the changes suggested to me, and it actually worked for a couple weeks. But, it reverted back. I tried for the last two months to fix this on my own, but I just can't figure it out. Does anyone have any idea what to do here? Incredibly appreciative of any assistance, Ruben

Wrong titles in site links
Hello fellow marketers, I have found this weird thing with our website in the organic results. The sitelinks in the SERP shows wrong written text. As in grammatically incorrect text. My question is where does Google get the text from? It is not the page title as we can see it. kKsFv0X.png

Google Is Indexing The Wrong Page For My Keyword
For a long time (almost 3 mounth) google indexing the wrong page for my main keyword.

The problem is that each time google indexed another page each time for a period of 4-7 days, Sometimes i see the home page, sometimes a category page and sometimes a product page.
It seems though Google has not yet decided what his favorite / better page for this keyword. In addition I have many links I get from bloggers that asked to do a review on one of my products, I'm very careful about that and so I'm always checking the blogger and their website only if it is something good, I allowed it. also i never ask for a link back (must of the time i receive without asking), and as I said, most of their links are anchor with my website name. I can't help but think something is wrong with my SEO
So we re-launched our site about a month ago, and ever since we've seen a dramatic drop in search results (probably due to some errors that were made) when changing servers and permalink structure. But, I can't help but think something else is at play here. When we write something, I can check 24 hours later, and if I copy the Title verbatim, but we don't always show up in SERPs. In fact, I looked at a post today, and the meta description showing is not the same, but when I check the source code, it's right.
