My crawl diagnostic is showing 2 duplicate content and titles.

My 1st question then:
When I view where these errors are occurring it says www mydomain co uk and www mydomain co uk/index.html
Isn't this the same page? I have looked into my root folder and only index.html exists.

That's perfect(and working) for redirecting all non www pages. You still need to decide on your original question regarding the index page!
Add the following to your htaccess file after the code you've already added :
RewriteCond %{THE_REQUEST} ^[A-Z]{3,9}\ /index\.html\ HTTP/ RewriteRule ^index\.html$ http://www.keystonemortgages.co.uk/ [R=301,L]

It's not essential but still useful to have the canonical tag, it doesn't redirect but tells the crawlers the preferred address if you have more than one page/address containing the same content. It's particularly useful for duplicate content like print versions, or results pages based on query strings.
What you've actioned could improve the rankings to your homepage as before, the search engines will have seen the two addresses as separate pages, therefore competing against each other. When the redirect comes into effect the page authority and other metrics of both will be combined into one.

Hi quick update:
Just this mean I won't need a canonical tag in my header? Do they do similar things?
I've created the htaccess file and dropped it into my root folder at 1and1 so I guess now I sit back for a re crawl to see if it works?
One more thing. Is this likely to effect rankings?

If you do not have a rewrite in place(in your htaccess file) then both the www.mydomain.co.uk AND www.mydomain.co.uk/index.html will resolve in your browser. I suggest doing a 301 rewrite and while you're at it make sure you also rewrite all non www varients -> eg http://mydomain.co.uk 301 redirect that to http://www.mydomain.co.uk
You can then go set your preferred version in your Webmaster Tools.
This would be the code for apache(check with your host/developer if you're unsure) :
RewriteEngine On
RewriteCond %{HTTP_HOST} ^example.co.uk
RewriteRule (.*) http://www.example.co.uk/$1 [R=301,L]

Duplicate Page Content on pages that appear to be different?
Hi Everyone! My name's Ross, and I work at CHARGED.fm. I worked with Luke, who has asked quite a few questions here, but he has since moved on to a new adventure. So I am trying to step into his role. I am very much a beginner in SEO, so I'm trying to learn a lot of this on the fly, and bear with me if this is something simple. In our latest MOZ Crawl, over 28K high priority issues were detected, and they are all Duplicate Page Content issues. However, when looking at the issues laid out, the examples that it gives for "Duplicate URLs" under each individual issue appear to be completely different pages. They have different page titles, different descriptions, etc. Here's an example. For "LPGA Tickets", it is giving 19 Duplicate URLs. Here are a couple it lists when you expand those:

1 page crawled ... and other errors
1. Why is only one (1) page crawled every second time you crawl my site? 2. Why do your bot not obey the rules specified in the robots.txt? 3. Why does your site constantly loose connection to my facebook account/page? This means that when ever i want to compare performance i need to re-authorize, and therefor can not see any data until next time. Next time i also need to re-authorize ... 4. Why cant i add a competitor twitter account? What ever i type i get an "uh oh account cannot be tracked" - and if i randomly succeed, the account added never shows up with any data. It has been like this for ages. If have reported these issues over and over again. We are part of a large scandinavian company represented by Denmark, Sweden, Norway and Finland. The companies are also part of a larger worldwide company spreading across England, Ireland, Continental Europe and Northern Europe. I count at least 10 accounts on Seomoz.org We, the Northern Europe (4 accounts) are now reconsidering our membership at seomoz.org. We have recently expanded our efforts and established a SEO-community in the larger scale businees spanning all our countries. Also in this community we are now discussing the quality of your services. We'll be meeting next time at 27-28th of june in London. I hope i can bring some answers that clarify the problem we have seen here on seomoz.org. As i have written before: I love your setup and you tools - when they work. Changing the way SEOmoz Detects Duplicate Content
Hey everyone, I wanted to highlight today's blog post in case you missed it. In short, we're using a different algorithm to detect duplicate pages. http://moz.com/blog/visualizing-duplicate-web-pages If you see a change in your crawl results and you haven't done anything, this is probably why. Here's more information taken directly from the post: 1. Fewer duplicate page errors: a general decrease in the number of reported duplicate page errors. However, it bears pointing out that: **We may still miss some near-duplicates. **Like the current heuristic, only a subset of the near-duplicate pages is reported. **Completely identical pages will still be reported. **Two pages that are completely identical will have the same simhash value, and thus a difference of zero as measured by the simhash heuristic. So, all completely identical pages will still be reported. 2. Speed, speed, speed: The simhash heuristic detects duplicates and near-duplicates approximately 30 times faster than the legacy fingerprints code. This means that soon, no crawl will spend more than a day working its way through post-crawl processing, which will facilitate significantly faster delivery of results for large crawls.

I got a long list of URLs indicating Content that is identical (or nearly identical) to content on other pages of your site forces your pages to unnecessarily compete with each other for rankings.
