Hierachy in a Title Tag Needed?

When coding the title tags of a website, is it important to note the hierarchy of the website so the search engine can find that page? So, for example, the title tag would be, for a subcategory:
Webpage Subcategory>Category>Website name
Does this help the search engine rankings at all? Or can the search engines figure out the hierarchy by reading a sitemap or the HTML readable navigation? Then you could focus on the descriptive keywords of the subcategory page for the title tag.
Also, should you always include the site name in the title tag?

My testing has not shown any improvements with regards to this effort - BUT you could try it, and share the results with the rest of us!
Best practice for SEO TITLES is something like:
Optimal Format
Primary Keyword - Secondary Keyword | Brand Name
or
Brand Name | Primary Keyword and Secondary KeywordSee this article on from the Moz team and a pretty good read too!

Do I need to re-index the page after editing URL?
Hi, I had to edit some of the URLs. But, google is still showing my old URL in search results for certain keywords, which ofc get 404. By crawling with ScremingFrog it gets me 301 'page not found' and still giving old URLs. Why is that? And do I need to re-index pages with new URLs? Do I put a canonical tag on the page I am pointing to?
Lets say B i a duplicate page of A (main page). I understand I have to put canonical tag under B to point to A. Do I also put canonical tag under the main page A? Is it necessary? I understand that A would then tell Google that it is preferred page of A? Is this a correct understanding?

H1 Page Title Tag Placement
I'm a little confused over the correct area to place a H1 title Tag. When I look at Wordpress templates and published Wordpress sites, it suggests placing the H1 tag within the header area. However, SEO companies and other well postioned sites place the H1 title tag at the start of the main content area. What is the correct and/or best practice for placing H1 page title tags? H2 Tag Backlink - is this safe?
I have found that my site is getting a link from a good site, but my concern is that the link is in a H2 tag in the footer of the front page of the site Would getting a link from a site wrapped in H2 tags be safe? The anchor is my sites brand name

Dropped 50 places for Page Title Change
Hey Guys, I've got a keyword set that is really annoying me. You see on the 19th July I changed my page title for the home page to what I thought would be something more competitive and instead of climbing from rank 9/10 I dropped out of the top 50. So I waited 2 weeks to see if it changed and it just stayed out of the top 50. After two weeks I changed it back again and I've been out of the top 50 ever since. I havnt made any real significant changes to the site as I've held off waiting for the keyword ranking to return but I dont think it will. I was trying to improve the keyword phrase "bahamas all inclusive" for the website www.smallhope.com. I'd managed to get on to page 1 rank 9/10 depending. I changed the title; from "Bahamas All Inclusive - Andros Bahamas Hotel - Bahamas Vacation - Small Hope Bay Lodge All-Inclusive" to "Bahamas All Inclusive Vacation - Small Hope Bay Lodge All-Inclusive" This is really bad for us and I'm not sure what to do. I had a whole plan in place starting with the Title and moving on to the H1 tags, but now I'm not so sure. I've put everything on hold. If this was you what would you do to get back to page 1? Tags, categories or both?
There is so much debate regarding duplicate content, horror stories, losing visitors, being penalized, yada yada... that I am wandering if it's wise to use tags/categories on a WordPress blog. I saw that all major blogs are using these structuring etiquettes and they are all dofollow and meta robots on index, follow. What do you say? It is wise to use tags, categories or both? Should I nofollow them, noindex or follow and index? Or noindex follow? Cheers and thx.
