Local Keywords Not Ranking Well in a Geographic Location (but Rank Very Well Outside of Geographic Location)

Has anyone experienced, in the last few months, an issue where a website that once ranked well for 'local' terms in Google stopped ranking well for those terms (but saw a ranking decrease only within the geographic location contained within those keywords)?
For example only, some 'root' keywords could be:
- Chicago dentist
- Chicago dentists
- dentist Chicago
- dentists Chicago
What happens is that when a searcher searches from within the geographic area of Chicago, IL, the target website no longer ranks on the 1st page for these types of keyword phrases, but they used to rank in the top 3 perhaps.
However, if someone was to search for the same keyword phrases from another city outside of Chicago or set a custom location (such as Illinois or even Milwaukee, WI perhaps) in their Google search, the target website appears to have normal (high) 1st page rankings for these types of terms.
My own theory:
At first I thought it was a Penguin related issue but the client's rankings overall haven't appeared to have been affected on the date(s) of Penguin updates. Authority Labs and Raven Tools (which uses Authority Labs data) did not detect any ranking decrease and still reports all the local keyword rankings as high on the 1st page of Google.
However, when the client themselves goes to check their own rankings (as they are within that affected geographic area), they are no where to be found on the 1st page. :S
After some digging I found that (one of) the company's Google Places listings (the main office listing) became an 'unsupported' status in Google Maps. So now I am thinking that this phenomenon is due to the fact that other listings are now appearing in search results for the same location.
For example, in this case, an individual dentist's Google Places listing (who works within the dental office) is being displayed instead of the actual dental office's listing. Also, the dentist's name on the Google Places listing is being swapped out by Google with the name of the dental office, but if you click through to the Google Places listing, it shows the name of the individual Dentist.
Anyone encounter a similar issue or have any other theories besides the Google Places issue?

Thanks Miriam, I'll take a look at those resources.
I actually have a correction to make in my original scenario. It is not that the place is 'unsupported', it is rather that it seems that one listing merged with another listing.
Thus the main office listing (along with 3 positive reviews) merged with one of the individual dentist listings.
It is worth mentioning that one Google rep did reply to me regarding another incident saying to check if the place was existent in Google Mapmaker. This opened up a whole other ballgame. If Google is using data from Map Maker, I can see why there are issues. That is a user driven database, verified by volunteer 'reviewers', etc...
I see some of the change histories for some of the places there a little scary. I think some 'users' don't realize that it is within Google's guidelines to have multiple listings for a single place if they are lawyers, dentists, etc.... or perhaps like some of the comments in the article links you posted mention... the listings are prone to competitor sabotage.

I have one keyword which disappeared
My site currently ranks for over 2000 of which 92 are in the number 1 position. Im very active with this site. My Main keyword is a two word keyword ,I have never been below the position #30. Has always been at best 3rd and fourth page. I rank primarily for mostly 3 part keywords and 4 part keywords. However , I noticed about a month ago,The main keyword no longer showing up on serp tools .Moz included. At first i was getting wierd results using keyword tool .Not moz serp tool. I would get different result every time i checked it. When i tried to verify these results ,Only to find my site not at the projected page and rank. After more than a month now this particular keyword is no where in site. Im still getting traffic fom all the other keywords but im a bit confused over this. I thought my main keyword was about to be on the bottom of the 2nd page but instead seems to be completely gone.

keywords are not ranking as per the expectation and need site review too
Hello, Recently We have a taken a project " http://customerconnect-services.com/ " and are responsible for the Digital Marketing for the website. FYI, we have been following the best On-Page SEO practices ever since we took the project: Performing Keyword Research, Finalizing the Keywords,Using those in the Page-Title, Meta-Descriptions, Heading Tags and of course in the Content as per MOZ suggestions & SEO standards. But, we are unable to rank in first page; all the considered keywords have a ranking of 51+ which is a serious matter of concern for us. We have also checked whether the Domain / URL has been blacklisted, but it's not (Not even by Google). We are therefore unable to figure out what is going wrong even after following so many best practices to get the keywords a good ranking (1st & 2nd page of SERPs). Therefore I would like to request you to provide your expert opinions in this regard by checking what is it that we are not getting right. The website url is http://customerconnect-services.com/. Drop in Rankings
Hello Webmasters, My site has incurred a sudden dip in rankings across sections. We conducted an analysis and have observed the following two major issues: Unnatural Links Penalty: Our site was issued an Unnatural Links Penalty on May 23. Basically, we have both 'http' and 'https' versions of our website registered on Webmaster tools. Initially, the warning showed up on the 'http' version and thus we started a cleanup by extracting the linking domains and have also filed a reconsideration request once all the spammy domains were removed and rightly disavowed. Recently, we got another manual action warning on the 'https' version regarding the unnatural links. So we have started with the cleanup activity right away. While analyzing this issue, we came across another major problem regarding the two versions which is our next concern and is mentioned below. https Canonical Issue: For more insights, we went through our site’s content and found that our website is following the below pattern Our 'http' version of the webpages get 301 redirected to the 'https' version. This 'https' version again has a canonical pointing to the 'http' version thus creating a loop. Difficulties in positiong a specific keyword
Hi, I am responsible for the blinds and curtains brand Kaaten and the its online store that operates in the Spanish market, during the last 6 months we have increasing substantially the traffic and improving the positioning of our main keywords (in most of the cases we postion right under Ikea and Leroy Merlin). However, there is a keyword that I cannot get to position, not even in the top 50 positions - that is the word "cortinas". I do not understand why is that happening as in a word like "estores" with similar monthly searches and competition we rank 3rd. Looking at the Home site and analysing both keywords the density is similar, so I do not see many differences there. I also cleaned up a bit the content to remove some "cortinas" keywords and thus avoid the keyword stuffing, but no results so far. Does any of you have any suggestion that what I can do to position better in that particular keyword? Look forward for your response and thanks for your help.

Ranking dropped after domain change

I use Moz Analytics for several days and try to figure out why our "main keyword" totally dropped after our domain change. We started this year with a new company name and a new domain. Our old domain was dp-hochzeitsfotograf.de and the main keyword we try to rank for was "hochzeitsfotograf" which means "wedding photographer" here in Germany. Last summer we reached position 1 and short before our domain change we still ranked on position 6. We register the domain change in google webmaster tools and redirect all urls via 301 redirect to the new domain. Our new domain name is yourheyday.de and all rankings adopted pretty good except our main keyword (now position 62) which could break our neck the next few month. I would be so so happy and really appreciate if anyone of you have an advise for us. Thanks and many greetings from europe

Localization without proper address?
Hi Mozzers, recently I received a project to promote a hotel website in a third world country. They have no street names, no landline phone, no zip-code. So far I tried to give a good address description in all social networks and on the homepage (footer) and signed into hotel directories. Suddently a new website of another hotel came up on google and made it up to number 1. They put a fake telefon number (landline) on the website. Is that a good idea of localizing a business? Do you have recommendations for me how to enhance. Thanks

Keyword stuffing found, how does Google treat my business name that contains keyword?
Hi, SEOmoz found 72 occurances of my keyword 'widgets' on our front page. I'm trying to cut it down but alot of the time the keyword appears alongside the company name 'Mega Widgets' . Will Google distinguish between the use of the keyword within a typical sentence and having it alongside the company name? Or should I put in a hypen (mega-widgets) or surround the company name with quotes ("mega widgets") to tell Google its a legit combination/phrase and I'm not trying to stuff in the keyword?

Google Webmaster Tools: Keywords
Hi SEOmozzers! I'm the Dr./owner/in-house SEO for my eye care practice. The URL is www.ofallonfamilyeyecare.com. Our practice is in O'Fallon, MO. Since I'm an optometrist, my main keywords are "optometrist o'fallon" and "o'fallon optometrist". As I get more familiarity with SEO, Google Analytics and Webmaster Tools, I've discovered the Keywords that Google feels best represent my website. About a week ago I noted Google counted 21 instances of "optometrist" on the 28-30 pages of my website, which ranks as #32 in the most common keywords. #1 is "eye" with 506 instances. Even though 21 occurrences seemed low, I went though every page adding "optometrist" a couple times in the body where it would naturally be appropriate. I also added it to the address shown on the footer of every page. I changed the top navigation option of "meet Dr. Hegyi" to "our optometrist". I must have added at least 4 occurrences to every page on my site, and submitted for a re-crawl. I even tried to scale back the "eye" occurrences on a few pages. Today I see that Google has re-crawled the site and the keywords have been updated. "Optometrist has DROPPED from #32 to #33. Does anyone have any ideas or suggestions why I'm not seeing increased occurrence in Googles eyes? I realize this may not be a big factor in SERPs, but every bit of on-page optimization helps. Or is this too minor of an issue to sweat? Thanks!
