Penguin update: Penalty caused from onsite issues or link profile?

Back in April before the Penguin update, our website home page ranked in the #1 position for several of our keywords and on page 1 for dozens of other keywords. But immediately after the Penguin update in April our rankings dropped immediately to below #100 for nearly all keywords. The sharp drop was obviously a penalty of some kind.
We worked on removing some bad back links that were questionable. Over the past 7 months many of the bad links have dropped off and our link profile is improving. Our rankings, however, have not improved at all. In Yahoo and Bing we remain strong and rank on page 1 for many of our keywords.
I joined SEOmoz because I’ve heard about their great tools and resources for SEO. The first thing I learned is that I had a lot of errors and warnings that need to be addressed and I’m optimistic that these items once addressed will get me out of that dreadful penalty box we’ve been in for 7 months now.
So with that quick summary of our SEO problems I have a few questions that I hope to get some direction on.
1. Crawl Diagnostics for my site in SEOmoz reports 7 errors and 19 warnings including missing meta description tags, temporary redirects, duplicate page content, duplicate page title, 4xx client error, and title element too long.
Could these errors and warnings be what has landed my website in some kind of penalty or filter?
2. A couple of the errors were duplicate page title and duplicate page content. So there appears to be a duplicate home page.

They are the same page but it looks like Google is seeing it as duplicate content.
Do I need to do a 301 redirect in the .htaccess file? I’m not sure how that would work since they are the same page. If that is possible how would I go about doing that?
3. Finally based on what I’ve described above is it more likely that the penalty we are experiencing is because of onsite issues or because of our link profile?
We would really appreciate any help or direction anyone can offer on these issues.

As you have not mentioned anything about receiving mail in Google Webmaster Tools account, I am assuming that you are hit by algorithm penalty. In that case, you need to get rid of as many spammy links coming from low quality article directories, directories, link pages and link networks. When it is done, you need to use Disavow Tools to include those spammy links where the link references still remain. You need to wait few weeks before you can start seeing any difference. _

Watch out for Google's over optimization penalty as you try and recover your ranks. Review all of your on site optimization and make any fixes as necessary to reduce the keyword density.

Going by your statements, it is highly likely the issue is your rankings are being suppressed by Google due to the Penguin update.
You stated "immediately after the Penguin update" your rankings dropped. Penguin was introduced on April 24th. Did your rankings drop on April 25th or 26th?
Have you or any agent working on your behalf (employee, developer, link builder, "seo") added any links to your site prior to April 2012? More specifically: directory links, forum posts, blog comments, link exchanges, link wheels, etc?
If the answer to the above two questions are yes, the definitive diagnosis would be your site is hit by Penguin. In that case, the issue is your site's backlinks. There may be other issues impacting your site, but the first issue which needs to be addressed is the backlinks.

Yes you need to use a redirect, you need to change your Apache. Htaccess

How can I stop a tracking link from being indexed while still passing link equity?
I have a marketing campaign landing page and it uses a tracking URL to track clicks. The tracking links look something like this: http://this-is-the-origin-url.com/clkn/http/destination-url.com/ The problem is that Google is indexing these links as pages in the SERPs. Of course when they get indexed and then clicked, they show a 400 error because the /clkn/ link doesn't represent an actual page with content on it. The tracking link is set up to instantly 301 redirect to http://destination-url.com. Right now my dev team has blocked these links from crawlers by adding Disallow: /clkn/ in the robots.txt file, however, this blocks the flow of link equity to the destination page. How can I stop these links from being indexed without blocking the flow of link equity to the destination URL?

Canonical homepage link uses trailing slash while default homepage uses no trailing slash, will this be an issue?
Hello, 1st off, let me explain my client in this case uses BigCommerce, and I don't have access to the backend like most other situations. So I have to rely on BG to handle certain issues. I'm curious if there is much of a difference using domain.com/ as the canonical url while BG currently is redirecting our domain to domain.com. I've been using domain.com/ consistently for the last 6 months, and since we switches stores on Friday, this issue has popped up and has me a bit worried that we'll loose somehow via link juice or overall indexing since this could confuse crawlers. Now some say that the domain url is fine using / or not, as per - https://moz.com/community/q/trailing-slash-and-rel-canonical But I also wanted to see what you all felt about this. What says you?

Crawl issues
Hello there, I have found that when crawling my site I have errors regarding the meta description and it says it is missing from few pages. I checked these pages but there is a meta description. I also ran the same report with other tools and it comes up the same issues. What should I do?

Duplicate content issue
Moz crawl diagnostic tool is giving me a heap of duplicate content for each event on my website... http://www.ticketarena.co.uk/events/Mint-Festival-7/ http://www.ticketarena.co.uk/events/Mint-Festival-7/index.html Should i use a 301 redirect on the second link? i was unaware that this was classed as duplicate content. Bad link profile?
Hi Mozzers! We have recently been handed this client due to the former SEO company building up a bad link profile, which resulted in the site dropping off the search results all together. Forcing them to get a new domain. This happened in July last year and we are unsure whether it would be wise to submit a reconsideration request and then 301 their old sites pages to the new domain. Basically I'm asking whether you can spot any spammy links being built in their profile. Rankings Issues
website: trophycentral.com Back in February of this year, we took a huge rankings hit on Google. We thought it might be Panda related, but realized it was before the big change. We also thought it could be related to our Mobile rollout and a wholesale system we put in place, but were not sure. Most pages were fine, but our home page was hit hard and taken off line, included for branded terms (although the branded terms came back in a couple of weeks). There were no manual actions against us. We made a ton of changes with the mobile site, crawl issues, reducing links, reviewing and removing potentially bad outside links, etc. After a few months (long time!) almost all of our keywords came back except one, trophies. We used to rank in the top few positions and now are not in the top 500. We believe we are being penalized, but have no idea why this one keyword is being impacted. It is obviously a huge one for us and we want to get it back. Does anyone have any ideas as to why Trophies is not ranking on our home page. We are grateful that we have many other words and phrases in the top 5 positions (not pages) of google, yahoo and bing, but on Google, we cant seem to get trophies to even show up! Please help!!!
