Simple on-site SEO - bet practice for keywords in content

Hello,
The Moz on-page grader will give a grade of A if the keyword appears exactly in the content at least one time.
If there are 500 words and a lot of it is about the main keyword, what have you found to be important to look for beyond the on-page grader - beyond the one exact instance of the keyword? I'm specifically talking just about keywords in the content.
My guess is that it needs to occur 3 or 4 times in different forms and at least once exactly, but the on-page grader doesn't require it.
What have you found?

Try not just focusing on the exact keyword but also closely related keywords. If the article is well written and picks up good social traction , you might end up ranking for different closely related keywords, not just the main keyword.

Interesting! Is this a hummingbird thing where it only need to be mentioned directly once as long as the content is on topic?

Matt is correct. Keyword density is no longer a thing that helps positively with ranks, in my opinion. It can hurt you if it is overused though.
If you're worried about content being naturally over-stuffed, that's a valid concern. If your client is in the pizza industry, how do you not use the word "pizza" all the time? The answer is: creatively. It'll be different with every industry, like with pizza, focus on toppings, dough, cheese, sauce, etc. to dilute the density.
Don't force it in, make it look natural, keep it in the H1, title, URL, and other best practices, but try to mention it only when required in the content to reduce mentions. Anything above one mention in the body isn't required, but sometimes quality of content requires it to be mentioned more.

We've found that "keyword density", as it used to be, isn't really a positive ranking factor any more. Like meta keywords, our testing shows that 1, 2, 3 times ... that's fine. 15, 20, 30 - that leads to problems. Anything else really doesn't matter. You can use it - don't overuse it. It's one of those things ... not really positive, just don't kill it to death. (We had a client put in a keyword over 500 times on one page. And didn't rank. SHOCKING.)

I usually try and have them in the Meta Title, Main Title ( preferably in H1 ), Sub titles as well. ( as long it makes sense to include them there )

HI Folks I'm monitoring a small Australian site bluetea.com.au . Currently I have a SEO specialist who does month onsite maintenance work for this site. However each month I continue to see errors in webmaster tools... as an example, currently webmaster tools suggest we have 21 short meta dic and 26 duplicate title tags..... Duplicate content issues - page content and store URLs
Hi, I'm experiencing some heavy duplicate content Crawl errors on Moz with www.redrockdecals.com and therefore I really need some help. It brings up different connections between products and I'm having a hard time figuring out what it means. It is listing the same products as duplicate content but they have different URL endings. URL SEO: Better directory structure vs. exact keyword phrase
I am trying to understand how to best optimise a url for a page to rank high for specific keywords. Example: a top keyword search is "rental properties in new york". Question is does this keyword need to appear as this exact phrase in the url or should it be broken up into different directories for a better structure e.g.: www.abc.com/en/properties/new-york/rental OR www.abc.com/en/rental-properties-in-new-york Which will help the page rank higher (given all other things on the page are exactly the same)? Breadcrumbs keyword repeats
Hi I have a client project who's developers platform is populating the category part of the breadcrumbs with the header tag. Since these include the pages primary target keywords/phrase they are being repeated in the breadcrumbs increasing the keyword/phrase count on the page as well as repeating/duplicating the sentence. Can this cause problems ? or not because Google knows its not part of the page content/body copy (because its a breadcrumb) ? Cheers Dan

Competitor Out Ranks My Site
Hi This is my first question on SEOMoz and I am confused how a competitor can out rank me for a local search term SEO Tamworth. I have a higher Page Authority, Domain Authority, Number of links etc.. Content by Country
Currently we have a news website aimed at several countries. We want to filter the content of some url (home, category pages, ...) using the country of origin of the visitor. For example in the home we've heard of global character, and a column with news of the country of origin of the visitor. This may affect the position or cause a Google penalty? thank you very much

Title Keyword Question
I'm writing up keywords for new pages on a website. There are a number of variations on the way we can say what we're looking for, and I don't want to post the specific keywords but I'll give an example using fruits. Let's say I want to optimize for Granny Smith apples, McIntosh apples, Jonathan apples, etc. Could my title be Apples - Granny Smith, McIntosh, Jonathan and my page will come up when someone searchs "Granny Smith apples" or "McIntosh apples" etc. or do the words have to be repeated in order. Obviously I will also be repeating these in the description and on the page I'm optimizing. Thanks!
