Page appearing multiple times in Warnings report

In reviewing my Moz warnings report, one page is appearing multiple times because the title is longer than recommended. Is this a bug in Moz?
The page is appearing with a number of different URLs, despite there being a rel="canonical" tag.
The page's canonical URL is:
http://betablog.org/wishing-and-hoping-and-praying/
And in the warnings report I'm seeing variations like this:
http://betablog.org/wishing-and-hoping-and-praying/?replytocom=26539
which are clearly links from the comments section.

Replytocom is a native WordPress error that can be a bit of a headache. I actually just answered a similar question about this a few days ago. that question was partially about Disqus but is still relevant. Here's the answer from that one:
Ah, Wordpress. The Replytocom parameter is an annoyance but can easily be fixed. Its not so much that removing Disqus is causing Replytocom... Disqus was just masking its presence. Replytocom exists as part of the native WordPress commenting system. Go to Webmaster Tools > Crawl > URL Parameters. Choose to Edit Replytocom, choose "Yes: Changes, reorders, or narrows page content", under How does it affect page content you can choose Other, and then choose to have Googlbot crawl No URLs with this parameter. You can also add replytocom to the Robots.txt for the blog to disallow it. And If you're using Yoast there's an option under SEO > Permalinks to remove ?replytocom variables. Getting Started: Multiple Campaigns VS Single Campaign
I'm new to Moz and am trying to determine whether to create multiple campaigns broken out into product categories or a single campaign and label keywords by product category. I figure the majority of the folks in the Moz Community are in agency space, so most probably prefer a single campaign per client. We're an in-house marketing team so we have the ability to track up to 10 campaigns but only need to track a single website. I like the idea of having the different campaigns for different product categories, but I don't want to make things overly complex either. Just because you can do something doesn't necessarily mean you should.

Why does a new page in Page Optimization say [No Title]?
I've just added a keyword and URL in Page Optimization. It gave me a 98 score - all good so far. But then when it added this page to the list of earlier pages it shows the keyword and URL but the title is [No Title] - and without a title I can't select it to edit it. Is this familiar to anyone?

How are keywords assigned to the On Page Rankings
Hopefully this isn't a stupid question. I'm new to MOZ Pro so I created a campaign and uploaded about 300 keywords. Our site is an ecommerce site so there are many many products and of course, many pages. My first On Page Ranking Report has completed and naturally we have pages with scores ranging from A to F. What I don't understand is this: for any given keyword, how did that word get assigned to a given page for the purpose of the ranking report? We may have many pages that may include a given keyword so when I look at a page with a score of F, I'm sometimes surprised that a given keyword is assigned to a particular page for the purposes of ranking. Can someone help me understand this architecture?

In Open site explorer the page title and Url show in the left hand column. Why do some of my pages have no data for page title?
I am a first time user. Newly updated site using Drupal and having lots of SEO problems. Under site explorer, several pages list NO DATA for the page title. This doesn't seem right. Any suggestions on what this means?

How does MOZ assign a keyword to a page in the On Page Grades by Keyword Ranking
I'm in the beta analytics tool. In the On Page Optimization > On Page Grades by Keyword section, there is a Keyword column with a keyword assigned to each URL on my site. http://analytics.moz.com/search/on-page/572409.326269 How is the keyword that is shown here decieed upon? Is it simply based on a guess of the "best keyword" for each page based on kwp density and other factors? Can I map a new keyword to each page? or at least to my top ten most important pages? I see I can check the grade for any keyword on any page here: http://analytics.moz.com/search/manage-page-grades/572409.326269. I want to map my top ten pages to ten keywords - can I save these changes in the system so I don't have to manually check a keyword/url match each time? Shaun
