How do I know for sure if my site has been slapped?

I'm new to this SEO business - and I focus on inbound marketing. My client's site is www.SubconsciousMind.com. Just a few weeks ago it was showing in the top search results for several major keywords. Now, it has disappeared all together and there are competitors showing that have very little SEO (metatarsi not set up properly, etc.). So, I know there has to be an opportunity.
Some obvious things:
There aren't a lot of links and the majority seem to be bad (bad linking farms)
Social media is set up by a robot
Articles are poorly written obviously ONLY for SEOMy client hired a SEO company awhile back to get results, not understanding black hat / white hat and it worked for several years. Now - it is really hurting her. The sites she is linking to doesn't have any contact info to get the "unlinked." I've read I can use the disavow tool.
I asked her if she got anything from Google about "being slapped". She doesn't even receive the emails to her site because she trusted someone else to set it all up.
Should I rebuild from scratch? Any recommendations? We are funning adwords now as a quick fix.

If you feel like you have done everything within your power to try and get the links removed, but there's just no way, then you should disavow the URL or domain. You should attempt to reach the owners of the domain 2-3 times before giving up. During my link removals, there have been a decent number of webmasters that finally responded on my 2nd or 3rd attempt.
As for disavowing URL or domain, if the entire domain is something you'd never want a link from, disavow the entire domain. Even if you only have ONE link from the entire site. Still, disavow the domain. Only disavow the URL if you think the site in general is good quality but you happen to be on 1 particular spammy page for some reason.

I checked with Google Webmasters and there is no penalty - good news! I've also attempted to contact those sites with spammy links and I am getting no response. So - would you recommend at this point that I simply go through the disavow at this point?

Philip gives good advise, I would add, that if there is no manual penalty, then the algorithm will be dismissing links at best, it would not be penalizing you for them. not for links anyhow
but as Philip said, the domain seems to be parked

How to see if your site has received a manual penalty from Google? Sign up for Google Webmaster Tools (http://www.google.com/webmasters).
From within there you will be able to see if the website has received a manual penalty from Google. Once you've got the account setup and verified, look on the left menu for "Search Traffic" and then "Manual Actions". If there is a MANUAL penalty imposed, you'll see it there. It will either be "partial" or "site-wide". The former means that only some pages are affected. The latter means that all pages are affected.
If you have a manual penalty, you will NEED to do outreach to get links removed, and THEN you submit a disavow for the ones you were unable to remove. You're right, most spam sites won't have contact info, but you can check http://www.who.is and find an email address in most cases. You will more than likely not get the result you're looking to get if you forego the link removal process and skip straight to a disavow.
One last note, if there's no manual penalty in Google Webmaster Tools, then the site may be penalized algorithmically. If you know there are spammy links in place, you should act as if you were penalized manually, with the only difference being that you won't be submitting a reconsideration request to Google. If the site quality is poor, you will want to research Panda recovery options.
By the way, www.SubconsciousMind.com doesn't seem to exist. So either you typo'd the name, or you have an ever bigger problem on your hands now

Spammy Website Framed My Site & Stole Rankings
Hi One of the pages of my website was starting to gather some real traction in Google rankings and hit 3500 visitors per day. This is the page: http://www.naturallivingideas.com/drinking-apple-cider-vinegar-benefits/ On 11th January search traffic to this page fell to virtually 0. The rest of the site rankings were unaffected. Yesterday I tried searching for some of the main keywords I was ranking for and instead of my search listing, this was appearing: Image: http://www.naturallivingideas.com/wp-content/uploads/2017/01/weird-rankings.jpg It is in the exact position I was ranking in, but instead this evn . moe site has stolen my ranking. Upon opening the website, it is simply my original article page in an iframe. If you look at the source code of the offending website, you will see what I mean. Hopefully now you are getting a 403 forbidden error as my host blocked referrals from that site but they still hold my rankings. Has anyone ever seen this before? How was this done? Do you see sites with unfixable Penguin penalties?
Hello, We have a site with 2 Penguin update penalties (drops in traffic) and one quality penalty (another drop in traffic) all years ago, both just drops in rankings and not messages in Google Console. Now that Penguin is hard coded, do you find that some sites never recover even with a beautiful disavow and cleanup? We've added content and still have some quality errors, though I thought they were minor. This client used to have doorway sites and paid links, but now is squeaky clean with a disavow done a month ago though most of the cleanup was done by deletion of the doorways and paid links 9 months ago. Is this a quality problem or is our site permanently gone? Let me know what information you need. Looking for people with a lot of experience with other sites and Penguin. Thanks.

Has our site been attacked?
Hello fellow mozers! I am having a problem you might be able to help me with and any thoughts on the issue will be greatly appreciated. Yesterday, I received an automated monthly report from Quill Engage, a tool that fetches data from Google Analytics and generates reports in a narrative format. Last month's 'referral traffic' section indicates two incredibly spammy websites driving more than 200 sessions to our website. Naturally, I checked out GWT and Open Site Explorer but couldn't find any traces of such activity. Futhermore, all our metrics seem ok. Can this possibly be a negative SEO attack that was only traced by the aforementioned tool? Can you propose any other way to test this and make sure we're not being attacked?

What do you say in your emails to horrible sites to remove your links?
Morning guys, I've the unenviable task of having to rectify poor link building (a previous company's work, not mine) which inevitably means emailing tons and tons of horrible directories with links to the client from as far back as 5/6 years ago. I'm sure many of you are in the same boat so it begs the question: What have you said to these types of sites that is effective in getting them to remove the links? This could even be a two/three-parter: If you've had little joy in requesting removals, have you dis-avowed the links, and what (if any) effect did it have? Thanks, M.

Penalty for all new sites on a domain?
Hi @all, a friend has an interesting problem. He got a manuel link penalty in the end of 2011...it is an old domain with domainpop >5000 but with a lot bad links (wigdet and banners and other seo domains, but nothing like scrapebox etc)...he lost most of the traffic a few days after the notification in WMT (unnatural links) and an other time after the first pinguin update in april´12. In the end of 2012 after deleting (or nofollowing) and disavow a lot of links google lifted the manuel penalty (WMT notification). But nothing happened after lifting, the rankings didn´t improve (after 4 months already!). Almost all money keywords aren´t in the top 100, no traffic increases and he has good content on this domain. We built a hand of new trust links to test some sites but nothing improved. We did in february a test and build a completely new site on this domain, it´s in the menu and got some internal links from content...We did it, because some sites which weren´t optimized before the penalty (no external backlinks) are still ranking on the first google site for small keywords. After a few days the new site started to rank with our keyword between 40-45. That was ok and as we expected. This site was ranking constantly there for almost 6 weeks and now its gone since ten days. We didn´t change anything. It´s the same phenomena like the old sites on this domain...the site doesnt even rank for the title! Could it still be an manuel penalty for the hole domain or what kind of reasons are possible? What do Bing and Yahoo look for in a site?
Do Bing and Yahoo look for authoritative sites like google does? Do they punish sites for black hat or spamming? The reason I ask these questions is because one of my competitors was ranking in first place for many great keywords in Google, they have the highest authority out of all of their competitors. They must have been punished by Google because now they are not ranking for any great keywords in Google. However they are ranking 1st in Bing and Yahoo for most the top keywords, getting the the most visibility out of all the sites. I attached a small Graph with latest visibility for the sites with the top keywords from google and then I also included the company that was punished from google they are the green circles on the graph. dIVI4.png

Problems with link spam from spam blogs to competitor sites
A competitor of ours is having a great deal of success with links from spam blogs (such as: publicexperience.com or sexylizard.org) it is proving to be a nightmare. Google does not detect these (the competitor has been doing well now for over a year) and my boss is starting to think if you can’t beat them, join them. Frankly, he is right – we have built some great links but it is nigh on impossible to beat 400+ highly targeted spam links in a niche market. My question is, has anyone had success in getting this sort of stuff brought to the attention of Google and banned (I actually listed them all in a message in webmaster tools and sent them over to Google over a year ago!). This is frustrating, I do not want to join in this kind of rubbish but it is hard to put a convincing argument against it when our competitor has used the technique successfully for over a year without any penalty. Ideas? Thoughts? All help appreciated

From page 3 to page 75 on Google. Is my site really so bad?
So, a couple of weeks ago I started my first CPA website, just as an experiment and to see how well I could do out of it. My rankings were getting better every day, and I’ve been producing constant unique content for the site to improve my rankings even more. 2 days ago my rankings went straight to the last page of Google for the keyword “acne scar treatment” but Google has not banned me or given my domain a minus penalty. I’m still ranking number 1 for my domain, and they have not dropped the PR as my keyword is still in the main index. I’m not even sure what has happened? Am I not allowed to have a CPA website in the search results? The best information I could find on this is: http://www.google.com/support/webmasters/bin/answer.py?hl=en&answer=76465 But I’ve been adding new pages with unique content. My site is www.acne-scar-treatment.co Any advice would be appreciated.
