Why do some Keywords collapse in SERPS?

I've been working my way up SERPS with some fairly uncompetitive keywords and have been comfortable with them in the 5-10 range. This week I have watch a number of them drop back to the mid 50s.
Why?

Repeat keywords on the pages/titles
I know it is important to avoid duplicate titles and title tags, but I wanted to verify. Lets say you are a collection agency, would it be smart as a strategy to do domain.com/collectionagency/Dallas_ collection_agency and have that same key word structure for many states so many pages?

Chasing a particular keyword (and failing)
Hello, Its probably the most common and annoying questions, but I'm pulling my hair out a bit (not helped by not really knowing what I'm doing). I'm trying to optimise this http://www.architecturaldecor.co.uk/collections/antique-door-handles collection page for the main keyword 'antique door handles' secondary keyword 'antique door knobs'. Mid February I changed the URL, title and description and it started to move up the rankings, I then tidied up the descriptions, titles, ALT tags for all the products in that collection and it moved up to #13 quite quickly. However over the last 10 days it dropped down page 2 and today it's down to page 3. Whats more frustrating is 90% of those ranked in front of me don't actually sell antique door handles. When I tidied up the products in that collection I was increasing using the term 'antique door handles'. Any suggestions would be really, really welcome, have I overdone 'antique door handles' in the product descriptions. Thanks in advance, Paul.

Designing path structure - readability or keyword density
We are looking at redesigning our URL structure to accommodate our expansion. This gives us a chance to change the path, but we have found conflicting advice on readability vs. keyword density. These are our three options. mywebsite.com/s/birmingham/restaurants (Keep it short so that the keywords dominate the path) or mywebsite.com/search/birmingham/restaurants (Accurately describe the content on the page) or mywebsite.com/top/birmingham/restaurants (Be a bit spammy and include a word often associated with our inbound searches) Does anyone have any experience on what works best?

Home Page Keywords not Ranking and Assigned to Inside Pages
Hi, thank you for taking the time to read this. We have a few websites with the same problem. I will use http://www.prepared-meals.com as an example: The home page was ranking on page one for keyword "Prepared Meals". The site is about 6 months old. We use the Moz page optimizer on all pages of our websites to score an A rating. Recently we found the home page is no longer showing up in search results and the keyword "prepared meals" now points to an inside page that is not relevant: http://www.prepared-meals.com/Senior-Meals/Moms-Meals-Reviews.html this page shows up for Prepared Meals around page 15 in Google results. We have read keywords in the URL might be the issue, even though the page optimizer in MOZ says to do that. We are wondering if this is the issue or there is some other problem we are not aware of. Google Webmaster Tools - Inappropriate Keywords
In December our website was the victim of a devastating pharma hack. Google Webmaster Tools never reported malware on our website, but our keywords within Webmaster Tools were all changed to keywords related to mens and ladies watches. Our Google organic search rank and resulting traffic has been dramatically impacted. Since realizing we were hacked, we rebuilt our website from the ground up with a clean and updated installation of Joomla CMS (from 1.5 to 2.5) and changed to a more secure server. As of December 21st, 2012, our website has been clean of the phama hack. We wrote Google and told them what happened. They replied with a "no manual spam actions were found" message. Unfortunately, Google Webmaster Tools still reflects all of the hacked keywords for mens and ladies watches. We are watching our Google Webmaster Tools everyday waiting for Google to remove the hacked keywords. It's been a little over a month and still only about 5 of the first 100 keywords have anything to do with our website. What do you recommend we do to expedite the removal of these inappropriate keywords in Google WMT?

Main Page Gone For Main Keyword
For the past 5-6 months I have consistenly ranked at positions #14-16 for snow guards on snoshield.com. The past 3 days I cannot find the home page anywhere in Google for that keyword. The only thing that has really changed over the past two months is I placed 3 guest blog posts on pretty highly trusted sites that are industry related and created links to the site using suggestions from getlisted. I've read other reports of others seeing similar things happen recently. I don't think this is a penguin thing, because I can still find the site by searching for the company name, I just can't find it when searching the keyword. I did notice that a different page on the site is now ranking in position #21 for this keyword, but this page is optimized for a different keyword phrase. Is it possible that even though the sub page is optimized for a different keyword phrase, I am cannibalizing the site?

Keyword stuffing ?
howdy people, I had a quick question about keyword stuffing. I am creating a page on my website and on the page i am going to have multiple links, around 15 or so. they all have the word "pickup line" which will be hyperlinked to the respected page. this page is going to serve as a sort of directory to all the various pick up lines, "cheesy", "funny", "dirty" etc. I have written some content on this page as well. I wanted to know that since the keyword "pickup" line will be showing up a lot ( in hyper link form) will this be considered keyword stuffing? for this page
