On page Analysis... how does seomoz choose to match a keyword to a url?

On my "On Page" analysis report it show me a couple pages that have D ratings... one key phrase is "my destination lodging" and it is matching it up with a url like mydomain.com/my-destination-hotels.html.
It says I'm ranking 37th for this term.... Is it just suggesting that I optimize this url because it is already ranking for that term?
So should I create a different page with a different url to better match this phrase?
Thanks for any ideas.

The on page analysis is showing you this URL as it is the one that you are currently ranking 37th for. The Grade applied to this page is determined by many on page elements that can be better optimised to improve your Grade - and in doing so the likelyhood of improving your rankings as a result.
With regard to better optimising your page to best match your keyword - I would suggest that you first (and foremost) look at focusing on first selecting a small number of potential keyword variations that are the most relevant to your niche, your content and your customer - make sure they are targeted keyword/s rather than a very broad keyword/s (especially if the broad keyword is one that is highly competitive.)
Then I would take my shortlist of keywords and do some keyword research using something like Google's Keyword Tool or similar and examine the popularity of these keyword phrases (I would also consider selecting phrase and exact matches for these keywords) to best determine the most popular variations based on the number of monthly searches.
Try and build a URL structure that matches this keyword too - for example www.mydomain.com/villas-in-florida/
(If this proves to be a different keyword to the one you are currently using and you want to change the URL structure, page title etc and some of the copy on page you should create a 301 redirect from the old URL to the new.)

The site is about one destination. I will create another page specifically for that term. Thanks

You probably can't create a separate page of unique content for every keyword variant of "my destination (lodging, motels, hotels, bed & breakfast, etc.). So, you must consider your business model, assess the competition, research your keywords, and create content pages that make the most sense.
If your site is about a single city them maybe you can get enough content and have enough time to cover all of the keyword variants. However, if your site covers every city and town in a state like California you will probably have a hard time coming up with all of that content and making it profitable.

Web Core Vitals and Page Speed Insights Not Matching Scores
We have some URLs that are being flagged as poor inside Search Console in the Web Core Vitals report. For example, the report is saying that some pages have too many CLS issues. When looking into things we can do to update we have noticed that when we run the same page using the PageSpeed Insights tool we are not getting the same bad scores. This is making it hard for us to actually know what needs to be addressed. Nor can we tell if a change actually fixed the issue because in PageSpeed Insights there is not an issue. "City page" links in footer of home page: Spammy?
Is listing a bunch of links to city pages in the footer of a home page considered "spammy" to Google? (ie- Chicago Alarms, Illinois Alarms, Naperville Alarms, etc.) What are the negative affects this might have on ranking, if any?

Duplicate Page Content on Empty Manufacturer Pages
I work for an internet retailer that specializes in pet supplies and medications. I was going through the Crawl Diagnostics for our website, and I saw in the Duplicate Page Content section that some of our manufacturer pages were getting flagged. The way our site is set up is that when products are discontinued we mark them as discontinued and use 301 redirects to redirect their URLs to other relevant products, brands, or our homepage. We do the same thing with brand and manufacturer pages if all of their products are discontinued. 90% of the time, this is a manual process. However, the other 10% of the time certain products come and go automatically as part of our inventory system with one of our fulfillment partners. This can sometimes create empty manufacturer pages. I can't redirect these empty pages because there's a chance that products will be brought back in stock and the page will be populated again. What can we do so that these pages won't get marked as duplicates while they're empty? Write unique short descriptions about the companies? Would the placement of these short descriptions matter--top of the page under the category name vs bottom of the page underneath where the products would go? The links in the left sidebar, top, and in the footer our part of our site architecture, so those are always going to be the same. Using fathead page keywords for directories and as a red herring to competitors
I'm fairly new to SEO and I have been reading a lot on here and the SEOmoz guides over the last few days, finding it very interesting. I am wondering about page keywords, I read that the engines no longer use them. In this thread they say they still use them because of directories. http://www.seomoz.org/ugc/why-i-still-use-meta-keywords#new-comment So I was wondering, because directories are always smaller scale than search engines would keywords that are normally high competition have more clout in them? If so then using them could be misleading to competition if they think they are the actual keywords? or might this contradict between your actual chosen keywords in the directories backlink anchor text or something?

Framed Pages and Dynamic Pages
Has anyone else had experience with different CMS's for Ecommerce . Ones that create static pages and others that dynamically create pages. What differences have you seen with rankings on google with the two. Spammy keywords on a page
My client's website has a box of text on each page that is spammy and horrible to read and stuffed with keywords. The text boxes are there only for search engines as they mean nothing to humans. I say remove them as it must be doing more harm than good. However, my client is scared to remove them as the text has been there on each page for ten years and he is worried about a drop in visitor numbers if they are removed. Is he right to be worried?
