Why a drop in certain keyword but not others?

Hi.
I am looking into a potential clients SEO issues and reasons why their rankings for specific keywords that they ranked for have dropped but others stayed the same or sometimes have improved.
They received a manual spam action that was revoked after some disavow and so on. In May of this year they noticed a huge drop in more generic terms.
The main keyword that was ranked 6th, dropped to 35th yet other keywords rose slightly.
I have noticed this issue for another potential client as well. They ranked 1st for their brand name, then received a spam action that was revoked. Now they do not rank for their brand name but do for other long tail keywords?
Any ideas the best way to investigate this or root out the issue and build to improve rank for more generic keywords?

Anchor text is where my thoughts lie.
I think there could be something here as they do have lots of exact match anchor terms for their brand name. plus one of the dropped keywords is that term. Not phrase but one word term.
Say their brand name is Gym Shack. They have dropped considerably for the term Gym whilst maintaining or improving for Gyms in England or MMA Gyms.
This is where I am leaning towards.
What can be done here if Google is devaluing the term Gym? Is it the links that need removing or does the focus need to shift somehow?

These type of scenarios are very hard to diagnose without specific details such as the text of the penalty and what the site is but let me give you a few reasons:

Anchor text way over done for the brand or for a main keyphrase. This can result in lowered rankings on that keyphrase while maintaining authority on others.

Specific penalty addressing these anchors, particular links that have now been devalued totally, etc. If you had a lot of links to "keyphrase 1" anchors and then Google devalued & penalised those links, you'd rank for other terms but not those.
If the brand name is fairly generic, overcooked anchors would be my first guess - but again, it's just a guess. There are probably 6-10 different potential causes of this.

How Important is it to Use Keywords in the URL
I wanted to know how important this measure is on rankings. For example if I have pages named "chair.html" or "sofa.html" and I wanted to rank for the term seagrass chair or rattan sofa.. Should I start creating new pages with the targeted keywords "seagrass-chair.html" and just copy everything from the old page to the new and setup the 301 redirects?? Will this hurt my SEO rankings in the short term? I have over 40 pages I would have to rename and redirect if doing so would really help in the long run. Appreciate your input.

Duplicate keywords in URL?
Is there such a thing as keyword stuffing URLs? Such as a domain name of turtlesforsale.com having a directory called turtles-for-sale that houses all the pages on the site. Every page would start out with turtlesforsale.com/turtles-for-sale/. Good or bad idea? The owner is hoping to capitalize on the keywords of turtles for sale being in the URL twice and ranking better for that reason.

Keyword Phrase vs. separate keywords - Title Tag best practices
Hello, What is your opinion about when to use a keyword phrase vs. 2 keywords, separated by a comma, in the title tag? For example, on this page, the title could be either: NLP Hypnosis, Language Patterns | Nlpca.com or NLP and Hypnosis Including Language Patterns | Nlpca.com Which do you guys think is best with respect to rankings, updates, and future updates?

From keyword rankings to ......... what KPI?
Hi Folks, I have a customer whos keyword rankings for Google have fluctuated rather widly over the past two months which has caused some consternation on their part dispite our reassurance. This is caused in large part due to their lack of understanding of SEO, little effort on their part in implementing changes for SEO and what I belive to be unrealistic expectations (having done no SEO work on their site wanting to see first page ranking for competitive keywords like 'heart attack' within 4-8 weeks). At the moment we are using just keyword rankings as a KPI and I wish to reframe them by using additional or alternative KPIs so that as rankings fluctuate with future Google seach algorithm tweaks and changes that the customer isn't solely focused on them. I am still in the process of formulating this list but so far I have decided to include the KPIs listed below. Month-on-Month / Quarter-On-Quarter Organic search traffic volume (should be rising) Top landing pages excluding branding keywords and homepage (should corelate to content created to target specific keywords) Number of landing pages on the client site that rank List of landing pages and bounce rates (are the 'gateway pages' holding visitors due to meeting their search requirements?) Average number of keywords per landing page (possibly integrated with the landig page reports above as a dimension to demonstrate correlation of # of keywords to landing pages) Some visibility on top keyword search terms (provided from Adwords where possible and GWT also) Top organic keywords (from adwords and GWT) Conversions from organic search (will vary from client to client for their own needs but will primarily be implemented using Google Tag Manager event tracking for things like enquiry forms etc) Referral traffic Delta/Ranking trends over large set of datapoints (will depend on how often you poll/track rankings but for example if you track rankings weekly then assess the trend of the rankings over 3-6months to smooth out the fluctuations) Your thoughts and feedback on this would be greatly appreciated. Hi , I just start one page website ( website that give a user one service like backlinkswatch . com or prchecker ) This niche is so competitive and the first 5 results for keywords that I have selected for my website have a google pagerank > 4 I know that ranking for high competitive keywords take more times but what Can I do to make it fast without getting penalized , all what I hear now is guest post and guest post and guest post any other idea ??

Help figuring out if certain paid directories are worth it
The person in my position previously had quite a few paid directories our site was listed on. What is the best resources you guys have used or know of to figure out which ones are good to keep? For instance one that is up for renewal this week is site-sift.com. I know the person previous to me did some not so ethical stuff and I'm trying to clean up messes. Any advice on directories would be much appreciated.

Keyword Density Question
Here's my hypothetical. I'm working on a car dealer site. And it's a Chrysler Jeep Dodge Ram dealer. Would "Chrysler Jeep Dodge Ram dealer," count as four keywords rather than one? My goal is to make the website show up for either Chrysler Dealer, Jeep Dealer, et cetera. Thanks!
