I have used my keyword only 12 times but on-page grader shows it 40 times?

I hope you are doing great, kindly have a look at this page, my keyword for this page is "Sports Streaming Sites". The text has a length of 5344 words and I'm using my keyword in this post as follow:
1. Text Body: 12 times(including headers)
2. Alt tags: 10 times (20+ Images)
3. Title: 1 Time
4. Description: 1 Time
5. Permalink: 1 TimeThis is a total of 25 times in 5344 words, but when I check this page with moz on-page grader, it shows that I'm using this keyword 40 times and its stuffed, can anybody please explain it to me, I'm so confused and it'll be so nice of you.

In this case Moz, isn't the smartest with their tool. What they're doing here is just checking the total source code apparently for the number of keyword matches in there. But what this does is that it's also checking all your META tags in the head. In your case I see at least another 4 mentions in there from OG:Description, OG:Title and Twitter:Title and Twitter:Description.
Another 4 mentions are currently happening in your Schema.org at the top for Organization, that's where I just stopped looking for more instances as they're probably just hidden somewhere else in either ALT tags or other snippets.
So I would, in this case, go with your own calculations for just using the number of mentions that is on the page and ignore the number that Moz is reporting here. Hopefully, Moz will change this over time and only check the occurrences in the actual BODY content.

Source page showsI have 2 h1 tags on my page. I can only find one.
When I grade my page it says I have more than one h1 tag. I view the source page and it shows there are two h1 headings with the same wording. If I delete the one h1 heading I can find, the page source shows I have deleted both of them. I don't know how to get to the other heading to delete it. And I'm off page one of google! Moz is treating my pages as duplicate content but the pages have different content in reality

Page Rank Report says #6 in Google but I can't find the page anywhere
So SEOMoz says that I've consistently ranked #6 for a certain keyword. But when I search I'm no where to be found. I've done regular searches, incognito and some non-seomoz reports and all come up with nothing in Google. I noticed it a week or two ago, but didn't think it would continue. This is no bueno. I wouldn't be surprised if I got penalized (luckily my homepage relatively well for similar keywords), an old seo consultant used very spammy tactics. I recently removed them, but not before I started to notice that I fell off the map. Why would SEOMoz not recognize this, and continue to say I'm ranking well? Using MOZ Keyword Rankings with Google Analytics in Excel to track paid keywords
Hi, I sorted my keywords in GA from highest incoming traffic to lowest, exported it and then imported these 280 or something keywords into SEO Moz. The problem is, it appears that SEO Moz Keyword Rankings tool pulls traffic data (in the traffic column) only for NON-PAID traffic ... So what I'm seeing is that keywords that brings A LOT of traffic to my site (because of AdWords) sometimes have 0 of traffic in SEO Moz, because I'm on page 3 of SERP and I literally got no non-paid traffic that week. The thing is: I want to improve my rankings for keywords that I'm already performing well, even if they are paid keywords, because they already bring me a lot of traffic to my website. The way this is working is not helping me out, because when I sort by the traffic column very important keywords end up in the last place for me. Questions: Whey does SEO Moz pull traffic data from non-paid keywords only ? Shouldn't be at least an option for this or I'm missing something here ? While they don't change it, any Excel Gurus could help me how to export SEO Moz ranking data, import it on Excel and MERGE a new "paid traffic" column that I export from Google Analytics ? How to assing each keyword line the paid traffic value I'm getting from GA ? Thanks ! Felipe

What do i do when all pages are grade A?
I've used the on page grade and now have all my pages at a grade A for relevant keywords. Most of them are cool, achieveing first page rankings apart from a few massive keywords. So the question is, what's next? What do i do now that I'm at grade A, but perhaps not #1 yet... Cheers -dan
