I have used my keyword only 12 times but on-page grader shows it 40 times?

I hope you are doing great, kindly have a look at this page, my keyword for this page is "Sports Streaming Sites". The text has a length of 5344 words and I'm using my keyword in this post as follow:
1. Text Body: 12 times(including headers)
2. Alt tags: 10 times (20+ Images)
3. Title: 1 Time
4. Description: 1 Time
5. Permalink: 1 TimeThis is a total of 25 times in 5344 words, but when I check this page with moz on-page grader, it shows that I'm using this keyword 40 times and its stuffed, can anybody please explain it to me, I'm so confused and it'll be so nice of you.

In this case Moz, isn't the smartest with their tool. What they're doing here is just checking the total source code apparently for the number of keyword matches in there. But what this does is that it's also checking all your META tags in the head. In your case I see at least another 4 mentions in there from OG:Description, OG:Title and Twitter:Title and Twitter:Description.
Another 4 mentions are currently happening in your Schema.org at the top for Organization, that's where I just stopped looking for more instances as they're probably just hidden somewhere else in either ALT tags or other snippets.
So I would, in this case, go with your own calculations for just using the number of mentions that is on the page and ignore the number that Moz is reporting here. Hopefully, Moz will change this over time and only check the occurrences in the actual BODY content.

Why would I be ranking for a term when it's not anywhere in my page or pages linking to it?
Hi, I have a keyword that's not anywhere in my page in page-grade tool nor are any pages linking to it with that term, why would that be ranking. The term is "dynamic card solution" This is a company we used to have 5 years ago and merged with our company. Inbound links not showing in reports
My clients site so far only has a few inbound links. If I do a report on MOZ/Open Site Explorer it only lists 10 domains linking in and, a number of important ones are not on that list. As an example, one of the leading UK national newspapers (Daily Telegraph) has a link on this page to the client www.vidavida.co.uk and yet this link/domain does not appear in the above reports. Is this an intentional nofollow type thing that the telegraph does and is there a way for me to check that on their site/page? http://fashion.telegraph.co.uk/news-features/TMG8655068/Holiday-travel-in-style.html Any comments most appreciated. Why are inbound links not showing?
I run the site http://www.eurocheapo.com and am finding that many inbound links are not showing up in OSE and on the toolbar. For example, check out this hotel review: http://www.eurocheapo.com/paris/hotel/hotel-esmeralda.html In OSE it shows only 2 links (from 1 domain), which is crazy. It has dozens of inbound links from many different domains (links:http://www.eurocheapo.com/paris/hotel/hotel-esmeralda.html). I notice this all over my site. Pages that we link between are also showing no internal links -- which is easy to disprove. Was there a problem with this crawl? Or is the problem in our code? Many thanks for your help, Tom

Duplicate page titles in SEOMoz
My on page reports are showing a good number of duplicate title tags, but they are all because of a url tracking parameter that tells us which link the visitor clicked on. For example, http://www.example.com/example-product.htm?ref=navside and http://www.example.com/example-product.htm are the same page, but are treated as to different urls in SEOMoz. This is creating "fake" number of duplicate page titles in my reports. This has not been a problem with Google, but SEOMoz is treating it like this and it's confusing my data. Is there a way to specify this as a url parameter in the Moz software? Or does anybody have another suggestion? Should I specify this in GWT and BWT?

Link not showing up in OSE
I created a profile in April this year on CrunchBase for my company http://www.crunchbase.com/company/wallpapered but it is not appearing in the "inbound links" of Open Site Explorer. All the other companies I have checked in OSE have their CrunchBase profile in their inbound links (many share the same Page authority as mine). Any suggestions would be really helpful. Thanks

No data showing in SiteExplorer
For this site: www.cooperator.biz/ I see no backlinks in SEO Moz Open SiteExplorer but Majestic SEO is showing 13 unique domains linking to it. I regularly encounter zero data for sites that I know have backlinks, why is this? If it's a problem with crawling them, why would I continue to use SEO Moz when other SEO services are showing me links? Kind regards, Rich
