Image Alt tags--always include the targeted keyword?

Question for all the SEO's out there. Do you always include your target keyword in the image alt tag?
For example, if you had an article on osteoarthritis, and you included a photo of an old man, would you put "old man on a bench" or "old man suffering from osteoarthritis" -- even though you have no idea if the old man suffers from osteoarthritis?

I have a site-wide banner on a website with a theme related to mine with the alt tag containing an exact-match keyword.
It's on 2,500 pages. Post-Penguin, am I inviting trouble? Should I play it safe and have it on, say, just a handful of pages -- or even just the homepage?

As a photographer in a past life, what I did that did not work was use the same tag on every image in a post. That doesn't work.
What does work is variety. Use the keyword, use related words, and use descriptive image words. Say you had 3 images to keyword:
(image of an older guy on a bench) alt="osteoarthritis frequently affects elderly men"
(doctor, any health image) alt="healthy screenings can help reduce the symptoms of osteoarthritis"
(drug you're selling) alt="prevent joint pain and reduce osteoarthritis symptoms with X"

Agree with Davinia 100%. Write alt tags for the user - how would you BEST describe the image in a few words? I try to include a few keywords/phrases but if I have a page with 10 images I'm not going to include the same keyword(s) in every single image - probably around 50-80% of the images.

I would use the keyword in a keyword phrase and limit to 2-3 words, so something like "Man with osteoarthritis" or "Osteoarthritis affects everyone".
Remember alt tags are also used for usability, so if someone say has a computer that reads them the on-page content (e.g. a visually impaired person) you want your content to clearly and accurately explain what's on the page.

H3 Tags - Should I Link to my content Articles- ? And do I have to many H3 tags/ Links as it is ?
Hello All, On my ecommerce landing pages, I currently have links to my products as H3 Tags. I also have useful guides displayed on the page with links useful articles we have written (they currently go to my news section). I am wondering if I should put those article links as additional H3 tags as well for added seo benefit or do I have to many tags as it is ?. ALT Tag Labels that Use Near Duplicate Text-SEO No, No???
Greetings Moz Community: About 280 pages of my 650 page commercial real estate website are listing pages. Each listing page contains between two and five photos, each with a corresponding ALT tag. My developer has set up the labeling of the ALT tags in the following manner. I can create a label for the first photo, but each subsequent photo automatically gets the same label plus a number tagged to the ALT. Like this: alt="Flatiron Loft for Rent"

alt="Flatiron Loft for Rent - Photo 3" Is this method neutral, positive or negative for SEO? I am concerned that this manner of labeling ALT tags might risk triggering a duplicate content penalty. In early July I migrated the site from Drupal to Wordpress. We changed the URL structure (adding a sub-directory) for the listings at that time. Google is refusing to index about 100 listing pages. Any chance the ALT tags are contributing to Google's reluctance to index the URLs? I might also add that images are hosted on Amazon's CDN. A sample listing URL is http://www.nyc-officespace-leader.com/listings/278-21st-street-flatiron-loft-for-rent

Exact keyword URL or not?
Hi all, I have a quick question about the proper use of permalinks. Let's say that I have a website about sports and I want to create an internal page dedicated to shoes. I know that the keyword "shoe" has 15.000 monthly visits, while the keyword "shoes" has 1.000 monthly visits. How do I have to name the internal page? http://www.example.com/shoe or http://www.example.com/shoes (with a final 's')? I would think that by naming the URL http://www.example.com/shoes, the search engine would consider that page for the keywords "shoe" and "shoes", but I am not sure about it. Should I create a URL that only focuses on one specific keyword ("shoe", in this example) or a URL that may encompass more than one keyword ("shoe" and "shoes")? I hope this is clear. Targeting Keywords at Home page or 301 URL?
Background info This is a new site and I am using Wordpress for a CMS not a blog and I have set a static page as the home page. The problem is when I configure the home page as a static page in wordpress it sets a 301 redirect to that page, which is this domain.com/software-consultancy (so the software-consultancy page is now domain.com). I thought about creating a separate Home page but I didn't see the point as the pages would be almost identical. **"Should I target keywords to the home page?" ** If I start link building using the phrase "software consultancy" to the home page should I link to domain.com or the 301 redirect URL domain.com/software-consultancy ? My thoughts are that if the URL has the keywords I am targeting in it will help with SEO. Or should I create a separate home page and just link the company name back to the home and have a separate page for "software consultancy" my thought is that having exactly what the company does on the home page would be better. But I don't want to lose out on search engine traffic by not having the keywords in the URL I am going to be link building for. I guess if I link build to the 301 redirect URL I can always revert back to this URL if I want to change? FYI here is my site structure: The site is for a small software consultancy and I have the following structure: domain.com/software-consultancy -- (Set as Wordpress static Page so essentially a 301 redirect to / ) domain.com/software-consultancy/areas domain.com/software-consultancy/areas/london domain.com/software-consultancy/areas/new-york I would really appreciate some feedback on this, even if it's experience / advice and there is no exact answer. Should I Decrease My Keyword Density
On SEOmoz's On-page tool it is telling me that I should cut the number of phrases for a specific key phrase almost in half, from 27 to 15. However my keyword density for this page is only 1.68%. I would have thought that was a good keyword density to have, if not even a little higher. Should I trust the tool and decrease this keyword phrase? Would love to hear from anyone with experience in this. If you want to see the page it is http://www.mybluedish.com/. The key phrase is "satellite internet"

Targetting site in 3 countries
I have read the seomoz post at - http://www.seomoz.org/blog/international-seo-where-to-host-and-how-to-target-whiteboard-friday before asking the question We recieved a query from one of our client regarding targetting his site in 3 different countries namely - US,UK and Australia. Specifically, he has asked us- 1. Whether i should buy ccTLD like - www.example.co.uk www.example.com.au www.example.com and write unique content for each of the above. or 2. or go for subfolder approach www.example.com/UK www.example.com/AU will it affect SEO if the subfolders are in CAPS. Would like to have advice of moz community on what advice will be the best. Thanks
