Image Alt tags--always include the targeted keyword?

Question for all the SEO's out there. Do you always include your target keyword in the image alt tag?
For example, if you had an article on osteoarthritis, and you included a photo of an old man, would you put "old man on a bench" or "old man suffering from osteoarthritis" -- even though you have no idea if the old man suffers from osteoarthritis?

I have a site-wide banner on a website with a theme related to mine with the alt tag containing an exact-match keyword.
It's on 2,500 pages. Post-Penguin, am I inviting trouble? Should I play it safe and have it on, say, just a handful of pages -- or even just the homepage?

As a photographer in a past life, what I did that did not work was use the same tag on every image in a post. That doesn't work.
What does work is variety. Use the keyword, use related words, and use descriptive image words. Say you had 3 images to keyword:
(image of an older guy on a bench) alt="osteoarthritis frequently affects elderly men"
(doctor, any health image) alt="healthy screenings can help reduce the symptoms of osteoarthritis"
(drug you're selling) alt="prevent joint pain and reduce osteoarthritis symptoms with X"

Agree with Davinia 100%. Write alt tags for the user - how would you BEST describe the image in a few words? I try to include a few keywords/phrases but if I have a page with 10 images I'm not going to include the same keyword(s) in every single image - probably around 50-80% of the images.

I would use the keyword in a keyword phrase and limit to 2-3 words, so something like "Man with osteoarthritis" or "Osteoarthritis affects everyone".
Remember alt tags are also used for usability, so if someone say has a computer that reads them the on-page content (e.g. a visually impaired person) you want your content to clearly and accurately explain what's on the page.

Is it better to optimise for several keywords/keyword variations on one page, or create sub categories for those specific terms?
I've done a fair of research to try to find the answer to this, but different people seem to give very different opinions, and none of the info I could find is recent! I'm working with a company that produces a range of industrial products that fit into 6 main categories, within this categories, there are types of products and the products themselves. Prior to my involvement most of the content was added to the product pages and very little was added to the overall category page. The structure works like this: Electronic devices > type of device > products The 'type of device' category could be something like a switch, but within that category are 3/4 different switch types...leaving me with 11 or 12 primary keyword/phrases to aim for as each switch is searched for in more than one way. Should I try to rank for all of those terms using that one category page? Or should I change the structure to something like: Electronic devices > type of device > sub-category/specific variation of device > product This would mean creating a page for each variation to have a more accute focus for a small number of phrases..but it also means I've added another step between the home page and the products. Any advice is welcome! I'm worried I'm overthinking it!

Alt image tags , shoud i reupload my site images as i never optimized right first time?

Would it be wise to reupload my sites images (100 in total) as I cannot rename the files on my wordpress site but it does allow me to add alt text , captions etc?Or just add the data i can to the images allready on the site? Would it be worthwhile in terms of search and pagerank?0 -
Are ALL duplicate title tags bad??
We’ve had some success recently by reducing the number of duplicate title tags on our website. We have managed to fix all the simple cases but there are a number of stubborn examples that we don’t know how to fix. A lot of the duplicate tags come from the website’s forums. Many questions have been asked multiple times over the years where the user has phrased the question in the same way. This has led to many cases where different forums posts have the same title tag. For example, there are six title tags with the words ‘’need help”! These are being highlighted as duplicates and currently we have several thousand of these. Would this be a problem? I’d be tempted to say that we should leave them as they don’t seem unnatural to me. One solution other solution we are considering is to append the forum name to the question to any post after the original, falling back to appending the date if that doesn’t distinguish it. Do people think that this is a good solution to implement or would it be better to leave these duplicate title tags as they are? Any help would be appreciated 🙂

Lazy loading images effect image seo?
Im using a wordpress plugin to lazy load images so that the site speed is a lot faster. Will this mess up image seo? Should I use the main keyword in the title tag for the site on all category pages?
I am pretty excited about changing all my title tags (for the most important 7 pages) since I have seen my rankings jump up in the SERP just by adding the main keyword for my website in the title tag. To make it easier I will explain my business. Simply, I run an online jewelry shop, so basically the keywords I want to use is "Jewelry online" and for the main categories "Necklace", "Rings" and "Bracelets". What I am unsure about is whether to use all the keywords in the main pages title tag or should I just use the main keyword "Jewelry online". I don’t want to create competition between my own pages of course. Jewelry Online - Trendy Fashion Jewelry | Homepage Or Jewelry Online - Necklace, Rings, Bracelets | Homepage And the same goes for the main categories, should I include "jewelry online" or not, like: Bracelets - Fashion Jewelry Online | Homepage Or Bracelets - Trendy_ Bangles_ and Arm Cuffs | Homepage Any suggestions what is the best practice for the title tag on main page and the main categories? Why should your title and H1 tag be different?
Is it dangerous to have your H1 tag and your title the exact same thing? My thought was that it's not be the best use of space, but that it couldn't cause harm. What do you think?
